If an employer has appointed an agent in advance, the employer may have that agent do the matters that the employer is required to do under this Ministerial Order.
事業主は、あらかじめ代理人を選任した場合には、この省令によつて事業主が行なわなければならない事項を、その代理人に行なわせることができる。
When an employer has appointed or dismissed an agent under the preceding paragraph, the employer must notify the Director of the Labor Standards Office Concerned or the Chief of the Public Employment Security Office Concerned to that effect by means of a notification stating the following matters. The same applies when a change arises in the matters stated in that notification that pertain to the appointment of the agent:
事業主は、前項の代理人を選任し、又は解任したときは、次に掲げる事項を記載した届書により、その旨を所轄労働基準監督署長又は所轄公共職業安定所長に届け出なければならない。当該届書に記載された事項であつて代理人の選任に係るものに変更を生じたときも、同様とする。

For a business for which the insurance relation pertaining to employment insurance has been established, the employment insurance applicable establishment number;
雇用保険に係る保険関係が成立している事業にあつては、雇用保険適用事業所番号
The employer's name or title, and address or location;
事業主の氏名又は名称及び住所又は所在地
Job title, name and date of birth of the agent who is appointed or dismissed;
選任し、又は解任する代理人の職名、氏名及び生年月日
Matters in which the agent acts;

Date of appointment or dismissal;

Name of the business and the place where the business is conducted.
事業の名称及び事業の行われる場所

前号の事務であつて、第三項第一号の事業に係るもの及び労働者災害補償保険（以下「労災保険」という。）に係る保険関係のみに係るもののうち、この省令の規定による事務 事業場の所在地を管轄する労働基準監督署長（以下「所轄労働基準監督署長」という。）
