第三十三条の二（法第二十二条第五項第二号の厚生労働省令で定める書類）
The documents specified by Order of the Ministry of Health, Labour and Welfare under Article 22, paragraph (5), item (ii) of the Act are as follows:
法第二十二条第五項第二号の厚生労働省令で定める書類は、次のとおりとする。
the wage ledger prescribed in Article 108 of the Labor Standards Act (Act No. 49 of 1947) or other documents proving that part of the wages is deducted as labor insurance premiums (meaning the labor insurance premiums prescribed in Article 10, paragraph (2) of the Labor Insurance Premium Collection Act; the same applies hereinafter);
労働基準法（昭和二十二年法律第四十九号）第百八条に規定する賃金台帳その他の賃金の一部が労働保険料（徴収法第十条第二項に規定する労働保険料をいう。以下同じ。）として控除されていることが証明される書類
among the withholding receipts prescribed in Article 226, paragraph (1) of the Income Tax Act (Act No. 33 of 1965) or the documents specified in Article 67, paragraph (1) of the Ordinance for Enforcement of the Corporation Tax Act (Ministry of Finance Order No. 12 of 1965), those proving that part of the wages is deducted as labor insurance premiums.
所得税法（昭和四十年法律第三十三号）第二百二十六条第一項に規定する源泉徴収票又は法人税法施行規則（昭和四十年大蔵省令第十二号）第六十七条第一項に定める書類のうち賃金の一部が労働保険料として控除されていることが証明されるもの
