When a qualified recipient who falls under Article 15, paragraph (4), item (iii) of the Act seeks to receive recognition of unemployment by submitting a certificate, the qualified recipient must submit a certificate of attendance at public vocational training, etc. (Form No. 15; hereinafter referred to as a "certificate of attendance" in this Section) to the Chief of the competent Public Employment Security Office.
