Supplementary Provisions, Article 1Effective Date
第一条（施行期日）
This Cabinet Order comes into effect on the date of promulgation.
この政令は、公布の日から施行する。
Supplementary Provisions, Article 2Definitions
第二条（定義）
In these Supplementary Provisions, the meanings of the terms listed in the following items are as prescribed respectively in those items.
この附則において、次の各号に掲げる用語の意義は、当該各号に定めるところによる。
"2006 Health Insurance Act, etc. Amendment Act" means the Act Partially Amending the Health Insurance Act, etc. (Act No. 83 of 2006);
平成十八年健保法等改正法 健康保険法等の一部を改正する法律（平成十八年法律第八十三号）をいう。
"New Health Insurance Order" means the Order for Enforcement of the Health Insurance Act as amended by this Cabinet Order;
新健保令 この政令による改正後の健康保険法施行令をいう。
"JHIA" means the Japan Health Insurance Association;
協会 全国健康保険協会をいう。
"single branch-insured person" means the single branch-insured person prescribed in Article 160, paragraph (1) of the Health Insurance Act;
支部被保険者 健康保険法第百六十条第一項に規定する支部被保険者をいう。
"premium rate for each prefecture" means the premium rate for each prefecture prescribed in Article 160, paragraph (2) of the Health Insurance Act;
都道府県単位保険料率 健康保険法第百六十条第二項に規定する都道府県単位保険料率をいう。
"transitional month of application" means in the case where the premium rate for each prefecture is changed, the first month in which the premium rate for each prefecture after the change (limited to that used on or before March 2020) is used;
経過措置期間適用月 都道府県単位保険料率の変更の場合における当該変更後の都道府県単位保険料率（平成三十二年三月以前に用いられるものに限る。）を用いる最初の月をいう。
"average premium rate" means the rate calculated, as specified by Order of the Ministry of Health, Labour and Welfare, in the same manner as under the provisions of Article 45-2 of the New Health Insurance Order (Article 45-3 of the New Health Insurance Order, where the transitional month of application is a month other than March), by deeming the estimated amount of the total sum of the amounts listed in Article 45-2, item (i) of the New Health Insurance Order in a business year to be the amount listed in that item, and the estimated amount of the total sum of the amounts listed in item (ii) of that Article in that business year to be the amount listed in that item;
平均保険料率 一の事業年度における新健保令第四十五条の二第一号に掲げる額の総額の見込額を同号に掲げる額と、当該一の事業年度における同条第二号に掲げる額の総額の見込額を同号に掲げる額とみなして、同条（経過措置期間適用月が三月以外の月の場合にあっては、新健保令第四十五条の三）の規定の例により厚生労働省令で定めるところにより算定した率をいう。
"highest premium rate for each prefecture" means the highest of the premium rates for each prefecture calculated, in a business year, pursuant to the provisions of Article 45-2 of the New Health Insurance Order (Article 45-3 of the New Health Insurance Order, where the transitional month of application is a month other than March);
最高都道府県単位保険料率 一の事業年度において新健保令第四十五条の二（経過措置期間適用月が三月以外の月の場合にあっては、新健保令第四十五条の三）の規定に基づき算定した都道府県単位保険料率のうち最も高い率をいう。
"fiscal 2009 transitional base rate" means the rate obtained by dividing by 10 the rate obtained by deducting the average premium rate in that fiscal year from the highest premium rate for each prefecture in fiscal 2009;
平成二十一年度経過措置基準率 平成二十一年度における最高都道府県単位保険料率から同年度における平均保険料率を控除した率を十で除して得た率をいう。
"item (i) average premium rate" means the rate calculated, as specified by Order of the Ministry of Health, Labour and Welfare, in the same manner as under the provisions of Article 45-2 of the New Health Insurance Order (Article 45-3 of the New Health Insurance Order, where the transitional month of application is a month other than March), by deeming the estimated amount of the total sum of the amounts listed in Article 45-2, item (i), (a) of the New Health Insurance Order in a business year to be the amount listed in item (i) of that Article, and the estimated amount of the total sum of the amounts listed in item (ii) of that Article in that business year to be the amount listed in that item;
第一号平均保険料率 一の事業年度における新健保令第四十五条の二第一号イに掲げる額の総額の見込額を同号に掲げる額と、当該一の事業年度における同条第二号に掲げる額の総額の見込額を同号に掲げる額とみなして、同条（経過措置期間適用月が三月以外の月の場合にあっては、新健保令第四十五条の三）の規定の例により厚生労働省令で定めるところにより算定した率をいう。

"item (ii) premium rate for each prefecture" means the rate calculated, as specified by Order of the Ministry of Health, Labour and Welfare, in the same manner as under the provisions of Article 45-2 of the New Health Insurance Order (Article 45-3 of the New Health Insurance Order, where the transitional month of application is a month other than March), by deeming the amount listed in Article 45-2, item (i), (b) of the New Health Insurance Order in a business year to be the amount listed in item (i) of that Article;
第二号都道府県単位保険料率 一の事業年度における新健保令第四十五条の二第一号ロに掲げる額を同号に掲げる額とみなして、同条（経過措置期間適用月が三月以外の月の場合にあっては、新健保令第四十五条の三）の規定の例により厚生労働省令で定めるところにより算定した率をいう。
"item (iii) premium rate for each prefecture" means the rate calculated, as specified by Order of the Ministry of Health, Labour and Welfare, in the same manner as under the provisions of Article 45-2 of the New Health Insurance Order (Article 45-3 of the New Health Insurance Order, where the transitional month of application is a month other than March), by deeming the amount listed in Article 45-2, item (i), (c) of the New Health Insurance Order in a business year to be the amount listed in item (i) of that Article;
第三号都道府県単位保険料率 一の事業年度における新健保令第四十五条の二第一号ハに掲げる額を同号に掲げる額とみなして、同条（経過措置期間適用月が三月以外の月の場合にあっては、新健保令第四十五条の三）の規定の例により厚生労働省令で定めるところにより算定した率をいう。
"rate equivalent to estimated revenue, etc." means the rate calculated, as specified by Order of the Ministry of Health, Labour and Welfare, in the same manner as under the provisions of Article 45-2 of the New Health Insurance Order (Article 45-3 of the New Health Insurance Order, where the transitional month of application is a month other than March), by deeming the amount specified by JHIA as the amount to be deemed revenue in the finances of that business year of health insurance on the basis of the branch-insured person concerned, out of the estimated amount of the reserves expected to be drawn down in a business year and other revenue for expenses required for health insurance business, to be the amount listed in Article 45-2, item (i) of the New Health Insurance Order;
収入等見込額相当率 一の事業年度において取り崩すことが見込まれる準備金の額その他健康保険事業に要する費用のための収入の見込額のうち当該支部被保険者を単位とする健康保険の当該一の事業年度の財政においてその収入とみなすべき額として協会が定める額を新健保令第四十五条の二第一号に掲げる額とみなして、同条（経過措置期間適用月が三月以外の月の場合にあっては、新健保令第四十五条の三）の規定の例により厚生労働省令で定めるところにより算定した率をいう。
"fiscal 2009 adjustment base rate" means the rate obtained by dividing by 10 the rate obtained by deducting the item (i) average premium rate in that fiscal year from the highest item (i) premium rate for each prefecture in fiscal 2009.
平成二十一年度調整基礎率 平成二十一年度における最高第一号都道府県単位保険料率から同年度における第一号平均保険料率を控除した率を十で除して得た率をいう。
Supplementary Provisions, Article 3
第三条
In fiscal 2013 and fiscal 2014, the phrase "the reserves expected to be drawn down in a business year and other revenue for expenses required for health insurance business" in item (xv) of the preceding Article is deemed to be replaced with "revenue for expenses required for health insurance business".
平成二十五年度及び平成二十六年度においては、前条第十五号中「一の事業年度において取り崩すことが見込まれる準備金の額その他健康保険事業」とあるのは、「健康保険事業」とする。
Supplementary Provisions, Article 4Time Limit for Transitional Measures Concerning Premium Rate for Each Prefecture
第四条（都道府県単位保険料率に係る経過措置の期限）
The date specified by Cabinet Order as referred to in Article 31 of the Supplementary Provisions of the 2006 Health Insurance Act, etc. Amendment Act is March 31, 2020.
平成十八年健保法等改正法附則第三十一条の政令で定める日は、平成三十二年三月三十一日とする。
Supplementary Provisions, Article 5Adjustment in Case of Change in Premium Rate for Each Prefecture
第五条（都道府県単位保険料率の変更の場合における調整）
In the case where a premium rate for each prefecture is changed, the criterion specified by Cabinet Order as referred to in Article 31 of the Supplementary Provisions of the 2006 Health Insurance Act, etc. Amendment Act for the calculation of that premium rate for each prefecture is the difference between the rate obtained by adding, to the average premium rate in the business year to which the transitional month of application belongs (where the transitional month of application is March, the business year following the business year to which that March belongs; the same applies hereinafter in this paragraph and paragraph (1) of the following Article), the transitional base rate for fiscal 2010 and thereafter, which the Minister of Health, Labour and Welfare specifies, taking into account the distribution of the premium rates for each prefecture before adjustment under the provisions of the following Article in the business year to which that transitional month of application belongs and the period from that transitional month of application through March 2020, within the range of the difference between the fiscal 2009 transitional base rate and the rate obtained by deducting the average premium rate in the business year to which that transitional month of application belongs from the highest premium rate for each prefecture in the business year to which that transitional month of application belongs, and 82/1,000.
都道府県単位保険料率の変更の場合における当該都道府県単位保険料率の算定についての平成十八年健保法等改正法附則第三十一条の政令で定める基準は、経過措置期間適用月の属する事業年度（経過措置期間適用月が三月の場合にあっては、当該三月の属する事業年度の翌事業年度。以下この項及び次条第一項において同じ。）における平均保険料率に、当該経過措置期間適用月の属する事業年度における次条の規定による調整前の都道府県単位保険料率の分布状況及び当該経過措置期間適用月から平成三十二年三月までの期間を勘案して、平成二十一年度経過措置基準率と当該経過措置期間適用月の属する事業年度における最高都道府県単位保険料率から当該経過措置期間適用月の属する事業年度における平均保険料率を控除した率との差の範囲内において、厚生労働大臣が定める平成二十二年度以降経過措置基準率を加えた率と千分の八十二との率の差とする。
The transitional base rate for fiscal 2010 and thereafter referred to in the preceding paragraph is to be set, as the rate to be applied for each business year from fiscal 2010 through fiscal 2019, by the end of the business year preceding each such business year, as a rate that is equal to or more than the transitional base rate for fiscal 2010 and thereafter referred to in that paragraph that was set as that to be applied in the business year preceding each such business year; provided, however, that the transitional base rate for fiscal 2010 and thereafter referred to in that paragraph that is to be applied in fiscal 2010 is a rate that is equal to or more than the fiscal 2009 transitional base rate.
前項の平成二十二年度以降経過措置基準率は、平成二十二年度から平成三十一年度までの各事業年度ごとに適用されるべき率として、それぞれ当該各事業年度の前事業年度末までに、当該各事業年度の前事業年度に適用されるべきものとして定めた同項の平成二十二年度以降経過措置基準率以上の率として定めるものとする。ただし、平成二十二年度に適用されるべき同項の平成二十二年度以降経過措置基準率は、平成二十一年度経過措置基準率以上の率とする。
Supplementary Provisions, Article 6
第六条
The adjustment under Article 31 of the Supplementary Provisions of the 2006 Health Insurance Act, etc. Amendment Act for the calculation of a premium rate for each prefecture in the case where that premium rate for each prefecture is changed is to be carried out, in accordance with the category of premium rates for each prefecture listed in the following items, as specified in those items.
都道府県単位保険料率の変更の場合における当該都道府県単位保険料率の算定についての平成十八年健保法等改正法附則第三十一条の規定に基づく調整は、次の各号に掲げる都道府県単位保険料率の区分に応じ、当該各号に定めるところにより行うものとする。

The adjustment base rate for fiscal 2010 and thereafter referred to in (b) of item (i) of the preceding paragraph is to be set, as the rate to be applied for each business year from fiscal 2010 through fiscal 2019, by the end of the business year preceding each such business year, as a rate that is equal to or more than the adjustment base rate for fiscal 2010 and thereafter referred to in (b) of that item that was set as that to be applied in the business year preceding each such business year; provided, however, that the adjustment base rate for fiscal 2010 and thereafter referred to in (b) of that item that is to be applied in fiscal 2010 is a rate that is equal to or more than the fiscal 2009 adjustment base rate.
前項第一号ロの平成二十二年度以降調整基礎率は、平成二十二年度から平成三十一年度までの各事業年度ごとに適用されるべき率として、それぞれ当該各事業年度の前事業年度末までに、当該各事業年度の前事業年度に適用されるべきものとして定めた同号ロの平成二十二年度以降調整基礎率以上の率として定めるものとする。ただし、平成二十二年度に適用されるべき同号ロの平成二十二年度以降調整基礎率は、平成二十一年度調整基礎率以上の率とする。
