The last day of the calculation period (計算期間), the period from 1 August each year to the following 31 July, on which high-cost medical expenses (高額療養費) and similar amounts for the whole period are worked out.

In Article 42 and Article 43, paragraph (1), the case where, in the 12 months up to the month of the treatment, high-cost medical expenses (高額療養費) under Article 41, paragraphs (1) to (4) have already been paid for three months or more; the base amount for calculating high-cost medical expenses (高額療養費算定基準額) is then lower.
A person who, on the reference date (基準日), is an insured person (被保険者) of this insurer, other than a day-labour special insured person (日雇特例被保険者), a member of a mutual aid association (共済組合) under the National Public Officers Mutual Aid Association Act or the Local Public Officers, etc. Mutual Aid Association Act, or a participant in the mutual aid system under the Private School Personnel Mutual Aid Association Act.
In Article 41, paragraph (5), Article 41-2 and Article 42, paragraphs (6) to (8), the treatment in Article 63, paragraph (1), items (i) to (iv) of the Health Insurance Act — examinations, medicines or materials, procedures, operations or other treatment, and management of treatment at home with the care that goes with it — other than treatment given with admission to a hospital or clinic.

The contribution that the Minister of Health, Labour and Welfare collects each fiscal year from day laborer health societies (日雇関係組合) toward the costs of health insurance for specially-insured day laborers (日雇特例被保険者), and which those societies must pay.
An insured person of the late-stage elderly medical care system (後期高齢者医療) under Article 50 of the Act on Assurance of Medical Care for Elderly People.
A hospital, clinic, pharmacy or other provider from which a person receives medical treatment.
In Article 41-2, outpatient treatment (外来療養) for which insurance benefits are given under Article 98, paragraph (1) of the Health Insurance Act (including as applied by Article 110, paragraph (7) and Article 111, paragraph (3)): the benefits that continue for treatment under way when a person loses insured status and becomes a day-labour special insured person (日雇特例被保険者) or the dependent of one.

当該支部被保険者
From Article 45-2 on, the branch-insured persons as defined in Article 160, paragraph (1) of the Health Insurance Act of the branch (支部) of the Japan Health Insurance Association (全国健康保険協会) in question: those employed at a place of business in that branch's prefecture and insured persons with optional and continued coverage (任意継続被保険者) living there.

前期高齢者納付金等
The payment for persons in the early stage of old age (前期高齢者納付金) and the related administrative expense contribution (前期高齢者関係事務費拠出金), which are collected from insurers each fiscal year under Article 36 of the Act on Assurance of Medical Care for Elderly People.
In Article 41-3 and Article 43-2, medical treatment for which insurance benefits are given under Article 98, paragraph (1) of the Health Insurance Act (including as applied by Article 110, paragraph (7) and Article 111, paragraph (3)): the benefits that continue for treatment under way when a person loses insured status and becomes a day-labour special insured person (日雇特例被保険者) or the dependent of one.
都道府県単位保険料率
The premium rate for each prefecture as defined in Article 160, paragraph (2) of the Health Insurance Act: the general premium rate (一般保険料率), between 3% and 13%, that the Japan Health Insurance Association (全国健康保険協会) decides separately for the insured persons of each prefectural branch (支部被保険者), and that applies to them.

後期高齢者支援金等
The support payment for persons in the late stage of old age (後期高齢者支援金), the related administrative expense contribution (後期高齢者関係事務費拠出金) and the administrative expense contribution related to childbirth and childcare (出産育児関係事務費拠出金) under the Act on Assurance of Medical Care for Elderly People.
The contribution for securing medical care in the early phase of an epidemic (流行初期医療確保拠出金) and the related administrative expense contribution (流行初期医療確保関係事務費拠出金), which insurers must pay under Article 36-14 of the Act on the Prevention of Infectious Diseases and Medical Care for Patients with Infectious Diseases.
The childbirth and childcare grant under Article 152-2 of the Health Insurance Act: money granted to insurers under Article 124-4, paragraph (1) of the Act on Assurance of Medical Care for Elderly People to meet part of the cost of the lump-sum allowance for childbirth and childcare (出産育児一時金) and the lump-sum allowance for dependent's childbirth and childcare (家族出産育児一時金).
A person in a condition of needing public assistance, whether or not they are currently receiving it.

In Article 5 of the supplementary provisions (附則) of Cabinet Order No. 357 of 2008, an insured person to whom Article 74, paragraph (1), item (ii) of the Health Insurance Act applies, or a dependent to whom Article 110, paragraph (2), item (i), (c) of that Act applies, who between January and March 2009 received treatment covered by other public medical benefits (特定給付対象療養) as defined in Article 32, paragraph (1) of the supplementary provisions of Cabinet Order No. 116 of 2008.

In the supplementary provisions (附則) of Cabinet Order No. 63 of 2009, the rate worked out, as a Ministry of Health, Labour and Welfare Order (厚生労働省令) provides, by treating a business year's estimated total of the amount in Article 45-2, item (i), (a) of this Order as that Cabinet Order amended it as the amount in item (i), and the estimated total of the amount in item (ii) as that amount, and following that Article (or Article 45-3, where the transitional month of application is not March).

In Article 43, paragraphs (1) and (5), the hospitals, clinics and pharmacies designated by the Minister of Health, Labour and Welfare (保険医療機関 and 保険薬局) and those listed in Article 63, paragraph (3), item (ii) of the Health Insurance Act — the hospitals, clinics and pharmacies an insurer designates to treat or dispense to the persons it insures — to which the insurer pays high-cost medical expenses (高額療養費) directly where the co-payment or other cost has not been paid.
In Articles 45-2 and 45-3, an insured person's standard monthly remuneration (標準報酬月額) and standard bonus amount (標準賞与額) added together.

In Articles 69 and 70, the business of benefits under Article 4, paragraph (1) of the supplementary provisions (附則) of the Health Insurance Act: benefits, up to the co-payment an insured person has paid, that an approved corporation formed by employers without a health insurance society and their insured employees may give those employees.

In Article 32 of the supplementary provisions (附則) of Cabinet Order No. 116 of 2008, an insured person to whom Article 74, paragraph (1), item (ii) of the Health Insurance Act applies, or a dependent to whom Article 110, paragraph (2), item (i), (c) of that Act applies, who between April and December 2008 received treatment covered by other public medical benefits (特定給付対象療養), limited to treatment for which a medical benefit the Minister of Health, Labour and Welfare designates for such persons is to be given.

前期高齢者交付金
In Articles 29 and 46, Article 65, paragraph (1), item (i), (a) and Article 67, paragraph (3), the grant for persons in the early stage of old age (前期高齢者交付金) that insurers receive under the Act on Assurance of Medical Care for Elderly People, which is taken off what a health insurance society (健康保険組合) spends on the payments and contributions it makes under that and other Acts.
平成二十四年一元化法
Short name for the Act Partially Amending the Employees' Pension Insurance Act, etc. to Unify, etc. the Employees' Pension Systems (Act No. 63 of 2012), the 2012 Act to unify the employees' pension systems (被用者年金制度).
The householder of a household that includes an insured person under National Health Insurance (国民健康保険), or a member of a National Health Insurance society (国民健康保険組合).
In Article 42, paragraph (12), items (ii) and (iii) and Article 43-3, paragraph (2), items (ii) to (iv), a person to whom Article 74, paragraph (1), item (iii) of the Health Insurance Act — the 30 percent co-payment for a person past the month they turn 70 whose remuneration is at or above the level a Cabinet Order (政令) sets — would apply if they received benefits for medical treatment (療養の給付) on the reference date (基準日).

In Article 43-2, the amount the Minister of Health, Labour and Welfare sets, having regard to the cost of handling payments of expenses for high-cost medical treatment combined with long-term care (高額介護合算療養費), by which a household's total must exceed the base amount before any such payment is made.
予定保険料納付率
In Articles 45-2 and 45-3, the rate, worked out as a Ministry of Health, Labour and Welfare Order (厚生労働省令) provides, of the premiums (保険料) expected to be paid in the following business year (事業年度) to the estimated amount of premiums to be collected for March of one business year through February of the next (for insured persons with optional and continued coverage (任意継続被保険者), April through March of the next business year), which the Japan Health Insurance Association (全国健康保険協会) uses in setting each prefecture's premium rate.

In Article 45-4, paragraph (4), the average benefit per person — the estimated cost for the following business year of benefits for medical treatment, etc. (療養の給付等) for all insured persons and dependents in the health insurance the Japan Health Insurance Association (全国健康保険協会) runs, less co-payments and the national subsidy under Article 153 of the Health Insurance Act, divided by their estimated number — multiplied by the estimated number of the branch's insured persons (支部被保険者) and their dependents, worked out as a Ministry of Health, Labour and Welfare Order (厚生労働省令) provides.

設立事業所
An applicable place of business (適用事業所) for which a health insurance society (健康保険組合) has been established; its employer and the insured persons employed there are members of that society.

The long-term care benefit and community support project support payment (介護給付費・地域支援事業支援納付金) collected each fiscal year from medical insurers (医療保険者) under Article 150 of the Long-Term Care Insurance Act.
子ども・子育て支援納付金
The children and child-rearing support levy that the government collects every fiscal year from 2026 from health insurers and similar bodies (健康保険者等) under Article 71-3, paragraph (1) of the Act on Support for Children and Child-rearing, whose payment a health insurance society (健康保険組合) may meet by drawing on its reserves (準備金) where its funds fall short.
The plan for putting its finances on a sound footing that a designated health insurance society (指定健康保険組合) must draw up under Article 28, paragraph (1) of the Health Insurance Act and have approved by the Minister of Health, Labour and Welfare, which this Order makes a three-year plan starting with the fiscal year after the year of the designation.
In Article 36-2, the running total of the injury and sickness allowance (傷病手当金), worked out under Article 99, paragraph (2) of the Health Insurance Act, that a person would receive from the day they become entitled to a disability allowance (障害手当金) under the Employees' Pension Insurance Act, as it stands on the day it reaches the amount of that disability allowance.

In Article 44-7, the grant (交付金) under Article 155-2 of the Health Insurance Act that the government pays the Japan Health Insurance Association (全国健康保険協会) without delay each time premiums, etc. (保険料等) are received into the health account (健康勘定): the amount received less the cost of the health insurance work the Minister of Health, Labour and Welfare carries out, worked out as a Ministry of Health, Labour and Welfare Order (厚生労働省令) provides.
In Article 45-3, paragraph (1), items (i) to (iii), the first month in which a changed premium rate for each prefecture (都道府県単位保険料率) is used.
In Article 5 of the supplementary provisions (附則) of Cabinet Order No. 241 of 2006, an insured person to whom Article 74, paragraph (1), item (iii) or Article 110, paragraph (2), item (i), (d) of the Health Insurance Act applies whose income under Article 34, paragraph (2) or Article 39, paragraph (2) of the Order for Enforcement of the Health Insurance Act, for treatment from September 2006 to March 2008, is under 6.21 million yen (4.84 million yen for one with no dependents); for them the base amount for calculating high-cost medical expenses (高額療養費算定基準額) is the amount in Article 42, paragraph (2), item (i) of that Order.
The date Cabinet Order No. 116 of 2008, the amending Cabinet Order these supplementary provisions (附則) belong to, comes into effect.
In Article 31 of the supplementary provisions (附則) of Cabinet Order No. 116 of 2008, the Order for Enforcement of the Health Insurance Act as it stood before its amendment by Article 1 of that Cabinet Order.
In Article 31 of the supplementary provisions (附則) of Cabinet Order No. 116 of 2008, a person listed in Article 42, paragraph (2), item (ii) of the Order for Enforcement of the Health Insurance Act whose income, for treatment from April to August 2008, is under 6.21 million yen (4.84 million yen for one with no dependents or former dependents), or who has no dependents but a former dependent and, for treatment from September to December 2008, would have income under 5.2 million yen counting that person as a dependent; for them the base amount for calculating high-cost medical expenses (高額療養費算定基準額) is the amount in Article 42, paragraph (2), item (i) of that Order as it stood before that Cabinet Order amended it.

生活療養
In Article 41, living support as defined in Article 63, paragraph (2), item (ii) of the Health Insurance Act: meals, and a suitable care environment as to temperature, lighting and water supply, given together with inpatient care to an insured person undergoing special long-term hospitalization (特定長期入院被保険者).
In Article 41, dietary treatment as defined in Article 63, paragraph (2), item (i) of the Health Insurance Act: meals provided as treatment together with inpatient care in a hospital or clinic, other than those given to an insured person undergoing special long-term hospitalization (特定長期入院被保険者).
A user of anonymized medical care-related information as defined in Article 150-3 of the Health Insurance Act: a person who has been provided with anonymized medical care-related information (匿名診療等関連情報) under Article 150-2, paragraph (1) of that Act and uses it, who pays the fee for its provision that Article 44-2 of this Order sets.

厚生労働省令で定めるところにより、年齢階級ごとに、当該年齢階級に係る年齢階級別平均一人当たり給付額（厚生労働省令で定めるところにより、療養の給付等のうち協会が管掌する健康保険の被保険者及びその被扶養者であって当該年齢階級に属する者に係るものについて当該一の事業年度の翌事業年度（第四十五条の二に規定する当該一の事業年度の翌事業年度をいい、前条の規定に基づき都道府県単位保険料率を算定する場合にあっては、同条に規定する当該適用月の属する事業年度をいう。以下この項において同じ。）に要する費用の見込額から当該見込額のうち法の規定により支払うべき一部負担金に相当する額及び法第百五十三条の規定による国庫補助の額の合算額の見込額を控除した額を、当該一の事業年度の翌事業年度において協会が管掌する健康保険の被保険者及びその被扶養者であって当該年齢階級に属する者の合計数の見込数で除して得た額をいう。）に当該一の事業年度の翌事業年度において当該支部被保険者及びその被扶養者であって当該年齢階級に属する者の合計数の見込数を乗じて得た額を合算した額
In Article 45-4, paragraph (4), the estimated total benefit cost (総給付費見込額) multiplied by the estimated total remuneration apportionment rate (総報酬按分率) under Article 160, paragraph (3), item (ii) of the Health Insurance Act for the following business year, worked out as a Ministry of Health, Labour and Welfare Order (厚生労働省令) provides.

From Article 63, paragraph (1), item (ii) on, the dispositions for arrears and other dispositions as defined in Article 204-2, paragraph (1) of the Health Insurance Act: the dispositions for arrears, etc. (滞納処分等) and the other powers under Article 204, paragraph (1), item (xvi) of that Act that the Minister of Health, Labour and Welfare has decided under paragraph (3) of that Article to exercise personally, with further powers a Ministry of Health, Labour and Welfare Order (厚生労働省令) names, which Article 204-2 lets the Minister delegate to the Minister of Finance.
年金事務所
A pension office (年金事務所) as provided in Article 29 of the Japan Pension Organization Act: an office the Japan Pension Service (日本年金機構) sets up where needed to handle part of the work of its branch offices, with an eye to the convenience of insured persons, employers and beneficiaries.
In Article 64-4, paragraph (1), items (iii) and (v) and Article 64-9, an employee of the Japan Pension Service (日本年金機構) appointed under Article 100-11, paragraph (2) of the Employees' Pension Insurance Act, as applied by Article 204-6, paragraph (2) of the Health Insurance Act, to receive the premiums and other money that Article 204-6, paragraph (1) of the Health Insurance Act lets the Service receive.
From Article 65, paragraph (1), item (i), (a) on, the benefits relating to medical care that each health insurance society (健康保険組合) provides, other than the additional benefits under Article 53 of the Health Insurance Act.
特定被保険者
From Article 71 on, a specified insured person as defined in Article 7, paragraph (1) of the supplementary provisions (附則) of the Health Insurance Act: an insured person who is not a category 2 insured person of long-term care insurance (介護保険第二号被保険者) but has a dependent who is, whose premium a health insurance society (健康保険組合) may set as the general premium, etc. amount (一般保険料等額) plus the long-term care insurance premium amount (介護保険料額).
同項に規定する特定被保険者
In Article 4 of the supplementary provisions (附則) of Cabinet Order No. 241 of 2006, the Order for Enforcement of the Health Insurance Act as amended by Article 2 of that Cabinet Order.
被扶養者
In Articles 4 and 5 of the supplementary provisions (附則) of Cabinet Order No. 241 of 2006, a dependent as provided in Article 39, paragraph (2) of the Order for Enforcement of the Health Insurance Act as that Cabinet Order amended it.
基準日とみなされる日
In Article 33 of the supplementary provisions (附則) of Cabinet Order No. 116 of 2008, a day that Article 43-4, paragraph (1) of the Order for Enforcement of the Health Insurance Act deems to be the reference date (基準日) provided in Article 43-2, paragraph (1), item (i) of that Order, in place of the last day of the calculation period.

In the supplementary provisions (附則) of Cabinet Order No. 63 of 2009, one tenth of the difference between the highest item (i) premium rate for each prefecture (最高第一号都道府県単位保険料率) and the item (i) average premium rate (第一号平均保険料率) for fiscal 2009.

都道府県単位保険料率
The premium rate for each prefecture as defined in Article 160, paragraph (2) of the Health Insurance Act: the general premium rate (一般保険料率), between 3% and 13%, that the Japan Health Insurance Association (全国健康保険協会) decides separately for the insured persons of each prefectural branch (支部被保険者), and that applies to them.
健康保険法第百六十条第二項に規定する都道府県単位保険料率
In Articles 3 and 19 of the supplementary provisions (附則) of Cabinet Order No. 240 of 2026, the Order for Enforcement of the Health Insurance Act as amended by Article 2 of that Cabinet Order, which comes into effect on 1 August 2027.
In Article 36, paragraph (1), items (i) and (ii), an accident in childbirth — limited to births meeting standards a Ministry of Health, Labour and Welfare Order (厚生労働省令) sets, and leaving out accidents from causes that Order names — through which the baby develops cerebral palsy and is left with a disability of the degree that Order sets.

七十五歳到達前旧被保険者
In Article 41, paragraph (4), a person who lost their status as an insured person under health insurance because, on reaching age 75 (Article 52, item (i) of the Act on Assurance of Medical Care for Elderly People), they became an insured person of the late-stage elderly medical care system (後期高齢者医療) on a day other than the first of a month.
In Article 41, paragraph (4), items (i) and (iii), the month in which a former insured person before reaching age 75 (七十五歳到達前旧被保険者) became an insured person of the late-stage elderly medical care system (後期高齢者医療), counting only the days of it up to the day that happened.
The period from 1 August each year to the following 31 July, over which annual amounts such as high-cost medical expenses (高額療養費) for outpatient treatment are worked out.

In Article 41-3, paragraph (2), the part of the amounts counted under paragraph (1) — co-payments (一部負担金) and other costs borne over the calculation period (計算期間) — that relates to medical treatment received from the month after the month a person turns 70, added together, less high-cost medical expenses (高額療養費) already paid and any money or goods given to lighten that burden; its excess over the annual base amount for persons aged 70 or over (七十歳以上年間高額療養費算定基準額) is paid to the insured person on the reference date (基準日被保険者) as high-cost medical expenses.
In Article 42, paragraph (1), item (v) and paragraph (3), item (v), a person on whom no municipal inhabitant tax (市町村民税) under the Local Tax Act — including the special ward inhabitant tax and leaving out the income-based levy imposed under Article 328 of that Act — is imposed for the fiscal year that includes the month of the treatment (the previous fiscal year where that month is April to July), including a person exempted from it by municipal ordinance and leaving out a person with no address where that Act applies on its assessment date (賦課期日).
療養のあった月の属する年度（療養のあった月が四月から七月までの場合にあっては、前年度）分の地方税法（昭和二十五年法律第二百二十六号）の規定による市町村民税（同法の規定による特別区民税を含むものとし、同法第三百二十八条の規定によって課する所得割を除く。第十項第二号、第十一項第四号及び第四十三条の三第一項第五号において同じ。）が課されない者（市町村の条例で定めるところにより当該市町村民税を免除された者を含むものとし、当該市町村民税の賦課期日において同法の施行地に住所を有しない者を除く。）
In Article 42, paragraph (11), item (iv) and paragraph (12), item (iv), a person on whom no municipal inhabitant tax (市町村民税) under the Local Tax Act is imposed for the fiscal year before the one that includes the reference date (基準日) — or, where a day from 1 August to 31 March of the previous year is deemed the reference date under Article 43, paragraph (12), for the fiscal year that includes that day — including a person exempted from it by municipal ordinance and leaving out a person with no address where that Act applies on its assessment date (賦課期日).

In Article 43, paragraphs (1) and (5), where the insurer pays the medical expenses combined with treatment outside insurance coverage (保険外併用療養費) straight to the hospital, clinic or pharmacy on the insured person's behalf, or treats them as paid, under Article 85, paragraph (5) or (7) of the Health Insurance Act as applied by Article 86, paragraph (4), the cost worked out for that treatment less the amount of those medical expenses: the part left for the insured person to pay.

In Article 43, paragraphs (1) and (5), where the insurer pays the medical expenses for home-nursing (訪問看護療養費) straight to the designated home-visit nursing provider on the insured person's behalf under Article 88, paragraph (6) of the Health Insurance Act, the cost worked out for that home-visit nursing less the amount of those medical expenses: the part left for the insured person to pay.

A day-labour special insured person (日雇特例被保険者, including a former one who still holds a handbook with space left for health insurance stamps, with some exceptions), an insured person under Mariners Insurance, a member of a mutual aid association (共済組合) under the National Public Officers Mutual Aid Association Act or the Local Public Officers, etc. Mutual Aid Association Act, a participant in the mutual aid system under the Private School Personnel Mutual Aid Association Act, a householder, etc. under National Health Insurance (国民健康保険の世帯主等), or an insured person of the late-stage elderly medical care system (後期高齢者医療).
