The matters specified by Order of the Ministry of Health, Labour and Welfare as referred to in Article 4-2, paragraph (2) of the Act are as follows:
法第四条の二第二項の厚生労働省令で定める事項は、次のとおりとする。
the name and address or location of the employer;
事業主の氏名又は名称及び住所又は所在地
the name of the business;
事業の名称
the place where the business is operated;
事業の行われる場所
the type of business; and
事業の種類
in the case of a business with a definite term, the scheduled period of the business.
有期事業にあつては、事業の予定される期間
The notification under Article 4-2, paragraph (2) of the Act must be made by submitting to the Director of the Labor Standards Office Concerned or the Chief of the Public Employment Security Office Concerned, within 10 days counting from the day following the day on which a change arose in a matter listed in the items of the preceding paragraph, a written notice stating the following matters:
法第四条の二第二項の規定による届出は、前項各号に掲げる事項に変更を生じた日の翌日から起算して十日以内に、次に掲げる事項を記載した届書を所轄労働基準監督署長又は所轄公共職業安定所長に提出することによつて行わなければならない。
the labor insurance number;
労働保険番号
the matter in which the change arose and the content of the change;
変更を生じた事項とその変更内容
the reason for the change; and
変更の理由
the date of the change.
変更年月日
When the written notice referred to in the preceding paragraph has been submitted, if the Director of the Labor Standards Office Concerned or the Chief of the Public Employment Security Office Concerned finds it necessary, the Director or Chief may request the employer to submit a certificate of registered matters or other documents by which the matters listed in the items of paragraph (1) can be confirmed.
所轄労働基準監督署長又は所轄公共職業安定所長は、前項の届書が提出されたときであつて、必要と認めるときには、事業主に対し、登記事項証明書その他の第一項各号に掲げる事項を確認できる書類の提出を求めることができる。

前号の事務であつて、第三項第一号の事業に係るもの及び労働者災害補償保険（以下「労災保険」という。）に係る保険関係のみに係るもののうち、この省令の規定による事務 事業場の所在地を管轄する労働基準監督署長（以下「所轄労働基準監督署長」という。）
