The requirements specified by Order of the Ministry of Health, Labour and Welfare as referred to in Article 9 of the Act are as follows:
法第九条の厚生労働省令で定める要件は、次のとおりとする。
that each business falls under only one of the following:

among the businesses for which the insurance relation pertaining to industrial accident insurance has been established, a business pertaining to the provisions of Article 39, paragraph (1) of the Act;
among the businesses for which the insurance relation pertaining to employment insurance has been established, a business pertaining to the provisions of Article 39, paragraph (1) of the Act; or
雇用保険に係る保険関係が成立している事業のうち法第三十九条第一項の規定に係る事業
a unified application business for which the insurance relations pertaining to industrial accident insurance and employment insurance have been established; and
that the businesses are of the same type of business.
それぞれの事業が、事業の種類を同じくすること。
An employer seeking to obtain the approval under Article 9 of the Act must submit a written application stating the following matters to the Director of the Prefectural Labor Bureau Concerned for the business that the employer wishes to have designated pursuant to the provisions of that Article:
法第九条の認可を受けようとする事業主は、次に掲げる事項を記載した申請書を、同条の規定による指定を受けることを希望する事業に係る所轄都道府県労働局長に提出しなければならない。
the name and address or location of the employer;
事業主の氏名又は名称及び住所又は所在地
the date of application;
申請年月日
the labor insurance number of the business that the employer wishes to have designated, the name of the business, the place where the business is operated, the insurance relation that has been established, and the type of business; and
当該指定を受けることを希望する事業の労働保険番号、当該事業の名称、当該事業の行われる場所、成立している保険関係及び当該事業の種類
among the businesses pertaining to the approval in question, for each business other than the business that the employer wishes to have designated, the labor insurance number of the business, the name of the business, the place where the business is operated, the insurance relation that has been established, and the type of business.
当該認可に係る事業のうち、当該指定を受けることを希望する事業以外の事業の労働保険番号、当該事業の名称、当該事業の行われる場所、成立している保険関係及び当該事業の種類
The designation under the provisions of Article 9 of the Act is to be made by the Director of the Prefectural Labor Bureau who received the application referred to in the preceding paragraph at the time of granting the approval under that Article for the application.
An employer who has obtained the approval under Article 9 of the Act must, if there has been a change in the name of a business, among the businesses pertaining to the approval, other than the business designated pursuant to the provisions of that Article, or in the place where the business is operated, without delay submit a written notice stating the following matters to the Director of the Prefectural Labor Bureau Concerned for the business designated pursuant to the provisions of that Article:
法第九条の認可を受けた事業主は、当該認可に係る事業のうち、同条の規定による指定を受けた事業以外の事業の名称又は当該事業の行われる場所に変更があつたときは、遅滞なく、次に掲げる事項を記載した届書を、同条の規定による指定を受けた事業に係る所轄都道府県労働局長に提出しなければならない。

労災保険率を定めた別表第一（労災保険率表）に掲げられている、事業の区分のことです。
都道府県労働局長
厚生労働省の地方支分部局である都道府県労働局の長で、都道府県ではなく国の機関の長です（厚生労働省設置法第十七条、国家行政組織法第九条）。労働基準監督官が充てられます（労働基準法第九十七条第二項）。労働基準法では、労働基準主管局長の指揮監督を受けて、管内の労働基準監督署長を指揮監督し、監督方法の調整その他労働基準法の施行に関する事務を行い、所属の職員を指揮監督します。下級官庁の権限を自ら行い、または所属の労働基準監督官に行わせることもできます（労働基準法第九十七条第一項・第九十九条第二項・第四項）。公共職業安定所長は、都道府県労働局長の指揮監督を受けて仕事を行います（職業安定法第八条第二項）。
