Article 50Reissuance of Qualified Recipient's Certificate or Notice of Recipient Qualification
第五十条（受給資格者証又は受給資格通知の再交付）
A qualified recipient who has lost or damaged the qualified recipient's certificate may notify the Chief of the competent Public Employment Security Office to that effect and receive reissuance. In this case, the qualified recipient must present a driver's license or other document that enables confirmation that the person intending to apply for reissuance of the qualified recipient's certificate is the person in question.
A person who intends to receive reissuance under the provisions of the preceding paragraph because of damage to the qualified recipient's certificate must submit the damaged qualified recipient's certificate.
The provisions of Article 17, paragraph (6) apply mutatis mutandis to the reissuance of a qualified recipient's certificate under the provisions of paragraph (1). In this case, the phrase "Chief of the Public Employment Security Office" in paragraph (6) of that Article is deemed to be replaced with "Chief of the competent Public Employment Security Office".
When effecting reissuance of a qualified recipient's certificate, if the Chief of the competent Public Employment Security Office finds it necessary, the Chief may temporarily postpone the decision on payment of the basic allowance.
The provisions of paragraph (1) apply mutatis mutandis to the reissuance of a notice of recipient qualification. In this case, the phrase "a driver's license or other document that enables confirmation that the person intending to apply for reissuance of the qualified recipient's certificate is the person in question" in that paragraph is deemed to be replaced with "Individual Number Card".
