When a qualified recipient who falls under Article 15, paragraph (4), item (iv) of the Act seeks to receive recognition of unemployment by submitting a certificate, the recipient must appear at the competent Public Employment Security Office on the first day of recognition of unemployment after the reason has ceased, and submit, attaching the qualified recipient's certificate (or, where the qualified recipient has been issued a notice of recipient qualification, presenting the Individual Number Card), a certificate from a public agency or a certificate of a person whom the Chief of the competent Public Employment Security Office finds appropriate, stating the matters listed in the following items:
the name and address or residence of the qualified recipient;
