法第三十七条の五第一項各号の要件を満たさなくなつた理由
in addition to what is listed in the preceding three items, matters specified by the Director-General of the Employment Security Bureau as necessary for the notification.
A person who makes the notification under the preceding paragraph must, where the reason for having ceased to satisfy the requirements of the items of Article 37-5, paragraph (1) of the Act is separation from service, attach to the written notification prescribed in the preceding paragraph, in addition to the documents prescribed in that paragraph, the documents specified in each of the following items according to the following categories of persons; provided, however, that this does not apply where the special elderly insured person does not wish to be issued a separation slip when the written notification prescribed in that paragraph is submitted.

a person listed in the items of Article 35 or a person who has separated from service for a reason listed in the items of Article 36: the documents prescribed in the preceding item, and documents that can prove that the person is a person listed in the items of Article 35 or has separated from service for a reason listed in the items of Article 36.
Notwithstanding the preceding two paragraphs, a person who makes the notification under paragraph (1) may, as specified by the Director-General of the Employment Security Bureau, omit attaching the documents prescribed in those paragraphs.
Notwithstanding the provisions of paragraph (1), when a special elderly insured person has ceased to be a special elderly insured person because of death or another unavoidable reason specified by the Director-General of the Employment Security Bureau, the employer that employs the special elderly insured person must submit the written notification prescribed in that paragraph within 10 days counting from the day following the day on which that fact occurred.
When an employer is requested by a person who intends to make the notification under the provisions of paragraph (1) to give the certification necessary for making that notification, the employer must give the certification promptly.
The provisions of Article 11 and Article 17 apply mutatis mutandis to the written notification prescribed in paragraph (1).
