第六十五条の十二（特例高年齢被保険者に対する休業等開始時賃金証明書の特例）
A special elderly insured person must, according to the case listed in each of the following items, by the day specified in that item, submit a certificate of wages at the commencement of leave, etc. to the Chief of the competent Public Employment Security Office, accompanied by a roster of workers, a wage ledger or other documents that can prove the day on which the leave was commenced, the amount of wages before that day and the period of employment. In this case, the provisions of Article 14-3, paragraph (1) do not apply to the employer that employs that special elderly insured person.
特例高年齢被保険者は、次の各号に掲げる場合に応じ、当該各号に定める日までに、休業等開始時賃金証明書に労働者名簿、賃金台帳その他の当該休業を開始した日及びその日前の賃金の額並びに雇用期間を証明することができる書類を添えて管轄公共職業安定所の長に提出しなければならない。この場合において、当該特例高年齢被保険者を雇用する事業主については、第十四条の三第一項の規定は、適用しない。

Notwithstanding the provisions of the preceding paragraph, a special elderly insured person may, as specified by the Director-General of the Employment Security Bureau, omit attaching the documents prescribed in that paragraph.
When the Chief of the Public Employment Security Office has received the certificate of wages at the commencement of leave, etc. pursuant to the provisions of paragraph (1), the Chief must issue to that special elderly insured person the wage certification slip at the commencement of leave, etc. prepared on the basis of that certificate of wages at the commencement of leave, etc.
公共職業安定所長は、第一項の規定により休業等開始時賃金証明書の提出を受けたときは、当該休業等開始時賃金証明書に基づいて作成した休業等開始時賃金証明票を当該特例高年齢被保険者に交付しなければならない。
When an employer is requested by a person who intends to make the notification under the provisions of paragraph (1) to give the certification necessary for making that notification, the employer must give the certification promptly.
