第百一条の二の十九（教育訓練休暇給付金の受給資格の決定）
A person who intends to receive payment of educational training leave benefits (excluding a person claiming unpaid benefits) must submit to the Chief of the competent Public Employment Security Office a written application for educational training leave benefits (Form No. 33-2-10), attaching a driver's license or other document capable of confirming that the person intending to receive payment of educational training leave benefits is the person themselves, documents capable of proving that the person obtained, before the leave commencement day, the employer's approval to take educational training leave (meaning educational training leave prescribed in paragraph (2) of the preceding Article; the same applies hereinafter), and an employment insurance certification slip of the monthly amount of wages at the commencement of educational training leave. In this case, where the person has been issued a written notice of extension of the benefit period for educational training leave benefits under Article 101-2-24, paragraph (5), the person must submit it together.
教育訓練休暇給付金の支給を受けようとする者（未支給給付請求者を除く。）は、運転免許証その他の教育訓練休暇給付金の支給を受けようとする者が本人であることを確認することができる書類、休暇開始日前に教育訓練休暇（前条第二項に規定する教育訓練休暇をいう。以下同じ。）を取得することについて事業主の承認を受けたことを証明することができる書類及び雇用保険被保険者教育訓練休暇開始時賃金月額証明票を添えて教育訓練休暇給付金支給申請書（様式第三十三号の二の十）を管轄公共職業安定所の長に提出しなければならない。この場合において、その者が第百一条の二の二十四第五項の規定により教育訓練休暇給付金受給期間延長通知書の交付を受けているときは、併せて提出しなければならない。
When the Chief of the competent Public Employment Security Office finds that a person who has submitted the written application for educational training leave benefits under the preceding paragraph falls under the main clause of Article 60-3, paragraph (1) of the Act (including where applied with the replacement under paragraph (3) of that Article; the same applies in Article 101-2-21), the Chief must determine the day on which that person is to receive the recognition that the person is taking educational training leave under Article 60-3, paragraph (4) of the Act (hereinafter referred to as "recognition of taking educational training leave") (hereinafter referred to as the "day of recognition of educational training leave"), inform that person of it, and enter the necessary matters in a notice of determination of recipient qualification for educational training leave benefits (Form No. 33-2-11) and issue it.
管轄公共職業安定所の長は、前項の規定により教育訓練休暇給付金支給申請書を提出した者が、法第六十条の三第一項本文（同条第三項において読み替えて適用する場合を含む。第百一条の二の二十一において同じ。）の規定に該当すると認めたときは、法第六十条の三第四項の規定によりその者が教育訓練休暇を取得していることについての認定（以下「教育訓練休暇取得の認定」という。）を受けるべき日（以下「教育訓練休暇取得認定日」という。）を定め、その者に知らせるとともに、教育訓練休暇給付金受給資格決定通知（様式第三十三号の二の十一）に必要な事項を記載した上、交付しなければならない。
