When a qualified recipient who falls under Article 15, paragraph (4), item (ii) of the Act seeks to receive recognition of unemployment by submitting a certificate, the recipient must appear at the competent Public Employment Security Office on the first day of recognition of unemployment after having interviewed a job offerer, and submit, attaching the qualified recipient's certificate (or, where the qualified recipient has been issued a notice of recipient qualification, presenting the Individual Number Card), a certificate of that job offerer stating the matters listed in the following items:
the name and age of the qualified recipient;
