Article 101-2-12Procedures for Application for Payment of Educational Training Benefits for Specified General Educational Training
第百一条の二の十二（特定一般教育訓練に係る教育訓練給付金の支給申請手続）
An educational training benefit recipient who intends to receive payment of educational training benefits pertaining to specified general educational training (hereinafter referred to as a "prospective trainee in specified general educational training" in this Article) must, no later than 14 days before the day on which the recipient starts that specified general educational training, submit to the Chief of the competent Public Employment Security Office an educational training benefits and educational training support benefits qualification confirmation form (Form No. 33-2-2), attaching the documents set out in the following items.
教育訓練給付金支給対象者であつて、特定一般教育訓練に係る教育訓練給付金の支給を受けようとするもの（以下この条において「特定一般教育訓練受講予定者」という。）は、当該特定一般教育訓練を開始する日の十四日前までに、教育訓練給付金及び教育訓練支援給付金受給資格確認票（様式第三十三号の二の二）に次の各号に掲げる書類を添えて管轄公共職業安定所の長に提出しなければならない。

When the Chief of the competent Public Employment Security Office finds that a prospective trainee in specified general educational training who has submitted the educational training benefits and educational training support benefits qualification confirmation form under the preceding paragraph is an educational training benefit recipient and falls under the persons set out in Article 101-2-7, item (ii), the Chief must give notice of the matters set out in the following items.
管轄公共職業安定所の長は、前項の規定により教育訓練給付金及び教育訓練支援給付金受給資格確認票を提出した特定一般教育訓練受講予定者が教育訓練給付金支給対象者であつて第百一条の二の七第二号に掲げる者に該当するものと認めたときは、次の各号に掲げる事項を通知しなければならない。

An educational training benefit recipient who falls under the persons set out in Article 101-2-7, item (ii) and has received the notice under the preceding paragraph must, when intending to receive payment of educational training benefits pertaining to specified general educational training, within one month counting from the day following the day on which the recipient completed the specified general educational training pertaining to payment of those educational training benefits, submit to the Chief of the competent Public Employment Security Office a written application for payment of educational training benefits (relating to Article 101-2-7, items (i) and (ii)), attaching the documents set out in the following items.
前項の規定による通知を受けた第百一条の二の七第二号に掲げる者に該当する教育訓練給付金支給対象者は、特定一般教育訓練に係る教育訓練給付金の支給を受けようとするときは、当該教育訓練給付金の支給に係る特定一般教育訓練を修了した日の翌日から起算して一箇月以内に、教育訓練給付金（第百一条の二の七第一号及び第二号関係）支給申請書に次の各号に掲げる書類を添えて管轄公共職業安定所の長に提出しなければならない。

An educational training benefit recipient who falls under the persons set out in Article 101-2-7, item (iii) and has received the notice under paragraph (2) must, when intending to receive payment of educational training benefits pertaining to specified general educational training, within one month counting from the day following the day on which the recipient completed that specified general educational training, made the acquisition, etc. of a qualification pertaining to that specified general educational training, and was employed as a generally insured person or an elderly insured person (for a person employed as a generally insured person or an elderly insured person, within one month counting from the day following the day on which the person completed that specified general educational training and made the acquisition, etc. of a qualification pertaining to that specified general educational training), submit to the Chief of the competent Public Employment Security Office a written application for payment of educational training benefits (relating to Article 101-2-7, item (iii)) (Form No. 33-2-3), attaching the documents set out in the following items.

Notwithstanding paragraphs (1) and (3) and the preceding paragraph, an educational training benefit recipient may, as specified by the Director-General of the Employment Security Bureau, omit attaching, among the documents set out in paragraph (1), item (iv), paragraph (3), item (v) and the preceding paragraph, item (v), those specified by the Director-General of the Employment Security Bureau.
The assigned career consultant is to conduct the career consulting referred to in paragraph (1), item (i) while paying attention to the following matters.
