Article 101-2-11Procedures for Application for Payment of Educational Training Benefits for General Educational Training
第百一条の二の十一（一般教育訓練に係る教育訓練給付金の支給申請手続）
When an educational training benefit recipient intends to receive payment of educational training benefits pertaining to general educational training, the recipient must, within one month counting from the day following the day on which the recipient completed the general educational training pertaining to payment of those educational training benefits, submit to the Chief of the competent Public Employment Security Office a written application for payment of educational training benefits (relating to Article 101-2-7, items (i) and (ii)) (Form No. 33-2), attaching the documents set out in the following items.
教育訓練給付金支給対象者は、一般教育訓練に係る教育訓練給付金の支給を受けようとするときは、当該教育訓練給付金の支給に係る一般教育訓練を修了した日の翌日から起算して一箇月以内に、教育訓練給付金（第百一条の二の七第一号及び第二号関係）支給申請書（様式第三十三号の二）に次の各号に掲げる書類を添えて管轄公共職業安定所の長に提出しなければならない。

Notwithstanding the preceding paragraph, an educational training benefit recipient may, as specified by the Director-General of the Employment Security Bureau, omit attaching, among the documents set out in item (iv) of that paragraph, those specified by the Director-General of the Employment Security Bureau.
教育訓練給付金支給対象者は、前項の規定にかかわらず、職業安定局長が定めるところにより、同項第四号に掲げる書類のうち職業安定局長が定めるものを添えないことができる。
