附　則

This Ministerial Order comes into effect as of April 1, 1974.

この省令は、昭和四十九年四月一日から施行する。

The written application for voluntary enrollment in Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance before amendment by this Ministerial Order (hereinafter referred to as the "Former Rules"), the written declaration of estimated insurance premiums, the written declaration of increased estimated insurance premiums and the written declaration of final insurance premiums in Form No. 6 (A) of the Former Rules, the notification of entrustment, etc. of processing of labor insurance administrative affairs in Form No. 17 of the Former Rules, and the notification of establishment of insurance relation in Form No. 21 of the Former Rules, which have been submitted at the time this Ministerial Order comes into effect, are deemed to be, respectively, the written application for voluntary enrollment in Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance after amendment by this Ministerial Order (hereinafter referred to as the "New Rules"), the written declaration of estimated insurance premiums, the written declaration of increased estimated insurance premiums and the written declaration of final insurance premiums in Form No. 6 (A) of the New Rules, the notification of entrustment, etc. of processing of labor insurance administrative affairs in Form No. 17 of the New Rules, and the notification of establishment of insurance relation in Form No. 21 of the New Rules.

この省令の施行の際現に提出されているこの省令による改正前の労働保険の保険料の徴収等に関する法律施行規則（以下「旧規則」という。）様式第一号による任意加入申請書、旧規則様式第六号（甲）による概算保険料申告書、増加概算保険料申告書及び確定保険料申告書、旧規則様式第十七号による労働保険事務処理委託等届並びに旧規則様式第二十一号による保険関係成立届は、それぞれ、この省令による改正後の労働保険の保険料の徴収等に関する法律施行規則（以下「新規則」という。）様式第一号による任意加入申請書、新規則様式第六号（甲）による概算保険料申告書、増加概算保険料申告書及び確定保険料申告書、新規則様式第十七号による労働保険事務処理委託等届並びに新規則様式第二十一号による保険関係成立届とみなす。

The written application for voluntary enrollment under Article 4, paragraph (1) of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance (hereinafter referred to as the "Rules"), the notification of entrustment, etc. of processing of labor insurance administrative affairs under Article 60 of the Rules, and the notification of establishment of insurance relation under Article 68 of the Rules may, for the time being, continue to be in the former form.

労働保険の保険料の徴収等に関する法律施行規則（以下「規則」という。）第四条第一項の規定による任意加入申請書、規則第六十条の規定による労働保険事務処理委託等届及び規則第六十八条の規定による保険関係成立届は、当分の間、なお従前の様式によることができる。
