Supplementary Provisions, Article 1Effective Date
第一条（施行期日）
This Ministerial Order comes into effect as of the date on which part of the Act Partially Amending the Employment Insurance Act, etc. comes into effect (October 1, 2010).
この省令は、雇用保険法等の一部を改正する法律の一部の施行の日（平成二十二年十月一日）から施行する。

第二条（労働保険の保険料の徴収等に関する法律施行規則の一部改正に伴う経過措置）
The identification card under Article 73 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance prior to the amendment by Article 2 (hereinafter referred to as the "former Labor Insurance Premium Collection Ordinance") is deemed, for the time being, to be the identification card under Article 75 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance as amended by Article 2 (hereinafter referred to as the "new Labor Insurance Premium Collection Ordinance").
第二条の規定による改正前の労働保険の保険料の徴収等に関する法律施行規則（以下「旧徴収則」という。）第七十三条の規定による証票は、当分の間、第二条の規定による改正後の労働保険の保険料の徴収等に関する法律施行規則（以下「新徴収則」という。）第七十五条の規定による証票とみなす。
A notification of establishment of insurance relation, a notification of entrustment of processing of labor insurance administrative affairs, etc. and a written application for voluntary enrollment in accordance with Form No. 1 of the former Labor Insurance Premium Collection Ordinance, a written application for approval as a labor insurance administrative affairs association in accordance with Form No. 16 of the former Labor Insurance Premium Collection Ordinance, a notification of termination of entrustment of processing of labor insurance administrative affairs, etc. in accordance with Form No. 17 of the former Labor Insurance Premium Collection Ordinance, and a notification of appointment or dismissal of an agent in accordance with Form No. 23 of the former Labor Insurance Premium Collection Ordinance, that have actually been submitted at the time of enforcement of this Ministerial Order, are deemed to be, respectively, a notification of establishment of insurance relation, a notification of entrustment of processing of labor insurance administrative affairs, etc. and a written application for voluntary enrollment in accordance with Form No. 1 of the new Labor Insurance Premium Collection Ordinance, a written application for approval as a labor insurance administrative affairs association in accordance with Form No. 16 of the new Labor Insurance Premium Collection Ordinance, a notification of termination of entrustment of processing of labor insurance administrative affairs, etc. in accordance with Form No. 17 of the new Labor Insurance Premium Collection Ordinance, and a notification of appointment or dismissal of an agent in accordance with Form No. 23 of the new Labor Insurance Premium Collection Ordinance.
A notification of establishment of insurance relation under Article 4, paragraph (2) of the new Labor Insurance Premium Collection Ordinance, a written application for approval as a labor insurance administrative affairs association under Article 63, paragraph (1) of the new Labor Insurance Premium Collection Ordinance, a notification of entrustment of processing of labor insurance administrative affairs, etc. under Article 64, paragraph (1) of the new Labor Insurance Premium Collection Ordinance, a notification of termination of entrustment of processing of labor insurance administrative affairs, etc. under Article 64, paragraph (2) of the new Labor Insurance Premium Collection Ordinance, a list of employers entrusting processing of labor insurance administrative affairs under Article 68, item (i) of the new Labor Insurance Premium Collection Ordinance, a book of collection and payment of labor insurance premiums, etc. under Article 68, item (ii) of the new Labor Insurance Premium Collection Ordinance, a register for the processing of notification affairs relating to insured persons of employment insurance, etc. under Article 68, item (iii) of the new Labor Insurance Premium Collection Ordinance, a notification of appointment or dismissal of an agent under Article 73, paragraph (2) of the new Labor Insurance Premium Collection Ordinance, a notice of establishment of the industrial accident insurance relation under Article 77 of the new Labor Insurance Premium Collection Ordinance, and a written application for voluntary enrollment under Article 2, paragraph (1) of the Supplementary Provisions of the new Labor Insurance Premium Collection Ordinance may, for the time being, continue to be in the former forms.
新徴収則第四条第二項の規定による保険関係成立届、新徴収則第六十三条第一項の規定による労働保険事務組合認可申請書、新徴収則第六十四条第一項の規定による労働保険事務等処理委託届、新徴収則第六十四条第二項の規定による労働保険事務等処理委託解除届、新徴収則第六十八条第一号の規定による労働保険事務処理委託事業主名簿、新徴収則第六十八条第二号の規定による労働保険料等徴収及び納付簿、新徴収則第六十八条第三号の規定による雇用保険被保険者関係届出事務等処理簿、新徴収則第七十三条第二項の規定による代理人選任・解任届、新徴収則第七十七条の規定による労災保険関係成立票並びに新徴収則附則第二条第一項の規定による任意加入申請書は、当分の間、なお従前の様式によることができる。
