第三十条の十三（氏に変更があつた者に係る住民票の記載事項の特例）
The matters specified by Cabinet Order prescribed in Article 7, item (xiv) of the Act, concerning the certificate of residence of a person whose surname has changed, are, in addition to what is provided for in Article 6-2, one former surname (meaning a surname that the person used in the past and that is entered or recorded in the family register or removed family register of the person; the same applies hereinafter in this Article and the following Article) and the kana reading of the former surname (meaning characters indicating how the characters used in the former surname are read and that are entered or recorded in the family register or removed family register of the person; the same applies in that Article) for which the person has requested entry in the certificate of residence under paragraph (1) or paragraph (4) of the following Article.
第三十条の十四（氏に変更があつた者の旧氏及び旧氏の振り仮名の住民票への記載等）
A person whose surname has changed (excluding a person who has a former surname and the kana reading of the former surname entered in the certificate of residence (hereinafter in this Article referred to as a "person with a former surname entered")), when seeking to have a former surname and the kana reading of the former surname entered in the certificate of residence, must submit a written request stating the former surname for which entry in the certificate of residence is sought, the kana reading of the former surname pertaining to that former surname, and other matters specified by Order of the Ministry of Internal Affairs and Communications to the mayor of a municipality that keeps the basic resident register in which that person is recorded (referred to as the "mayor of the municipality of domicile" in the following paragraph and paragraphs (4) through (6)). In this case, if a former surname and the kana reading of the former surname have ever been entered in the certificate of residence of that person, the person may seek the entry in the certificate of residence only of the former surname that the person has used on or after the day on which the former surname and the kana reading of the former surname that had been entered in the certificate of residence of that person were last deleted, and the kana reading of the former surname pertaining to that former surname.

Where the person making the request under the preceding paragraph is a person who does not have a registered domicile within the area of the municipality, and the mayor of the municipality of domicile cannot confirm, through the procedure under Article 10-2, paragraph (2) of the Family Register Act (Act No. 224 of 1947) (including as applied mutatis mutandis pursuant to Article 12-2 of that Act) carried out pursuant to Article 120-2, paragraph (1) of that Act (referred to as the "in-office confirmation procedure" in paragraph (5)), that the former surname and the kana reading of the former surname pertaining to the request are that person's former surname and the kana reading of the former surname pertaining to that former surname, the mayor may request that person to submit a family register confirmation document certifying this (meaning a certified copy of the family register, etc. prescribed in Article 10, paragraph (1) of that Act, a certified copy of the removed family register, etc. prescribed in Article 12-2 of that Act, or any other document specified by Order of the Ministry of Internal Affairs and Communications; the same applies in paragraph (5)).

In the cases listed in the following items, when the mayor of a municipality makes an entry in the certificate of residence of a person whose surname has changed, the mayor must enter the former surname and the kana reading of the former surname specified in the relevant item in the certificate of residence of that person.

When the surname of a person with a former surname entered has changed, that person may request to change the former surname and the kana reading of the former surname entered in the certificate of residence of that person to the former surname that the person used immediately before that change and the kana reading of the former surname pertaining to that former surname. In this case, the person must submit a written request stating that former surname and the kana reading of the former surname and other matters specified by Order of the Ministry of Internal Affairs and Communications to the mayor of the municipality of domicile.
Where the person making the request under the preceding paragraph is a person who does not have a registered domicile within the area of the municipality, and the mayor of the municipality of domicile cannot confirm, through the in-office confirmation procedure, that the surname of that person has changed, that the person used the former surname pertaining to the request immediately before that change, and that the kana reading of the former surname pertaining to the request is the kana reading of the former surname used immediately before that change, the mayor may request that person to submit a family register confirmation document certifying these matters.
A person with a former surname entered, when seeking deletion of the former surname and the kana reading of the former surname entered in the certificate of residence of that person, must submit a written request stating that deletion is sought and other matters specified by Order of the Ministry of Internal Affairs and Communications to the mayor of the municipality of domicile.
The provisions of Article 27, paragraphs (2) and (3) of the Act apply mutatis mutandis to the requests under paragraph (1), paragraph (4) and the preceding paragraph.
With regard to the application of the Act and the provisions of this Order to a person with a former surname entered, the phrases listed in the middle column of the following table in the provisions listed in the left-hand column of the table are deemed to be replaced with the phrases listed in the right-hand column of the table.

One is made for each married couple and their children who share their surname, where they set their registered domicile (本籍); when a new register is made for a person married to a foreign national (外国人) or with no spouse, it is made for that person and their children who share their surname (Family Register Act, Article 6).
