一Notification under Article 22 of the Act (hereinafter referred to as "moving-in notification") (excluding the notification listed in item (iii)), notification under Article 30-46 of the Act, and notification under Article 30-47 of the Act (excluding the notification listed in item (iv)): the following matters;

イAn indication that the person has acquired qualification as an insured person under National Health Insurance;

ロOccupation;

ハIf there is already a person who has acquired qualification as an insured person under National Health Insurance in the household to which the person has come to belong, the insured person's symbol and number (meaning the insured person's symbol and number prescribed in Article 111-2, paragraph (1) of the National Health Insurance Act; the same applies hereinafter in this Article) pertaining to any one of the insured persons belonging to that household;

ニIn the case prescribed in (c), if the householder of that household is subject to the application of Article 54-3, paragraph (1) or the main text of paragraph (2) of the National Health Insurance Act, an indication to that effect;

二Notification under Article 23 of the Act (hereinafter referred to as "notification of a move within a municipality" in this Chapter and Article 30-19), moving-out notification, and notification under Article 25 of the Act (referred to as "notification of household change" in item (ii) of the following Article and item (ii) of Article 27-3): the following matters;

イThe insured person's symbol and number pertaining to the person;

ロIf the householder of the household to which the person belongs is subject to the application of Article 54-3, paragraph (1) or the main text of paragraph (2) of the National Health Insurance Act, an indication to that effect;

三Moving-in notification (limited to one pertaining to changing an address within the area of a prefecture): the following matters;

イThe date on which the person acquired qualification as an insured person under National Health Insurance;

ロIf there is already a person who has acquired qualification as an insured person under National Health Insurance in the household to which the person has come to belong, the insured person's symbol and number pertaining to any one of the insured persons belonging to that household;

ハIn the case prescribed in (b), if the householder of that household is subject to the application of Article 54-3, paragraph (1) or the main text of paragraph (2) of the National Health Insurance Act, an indication to that effect;

四Notification under Article 30-47 of the Act (limited to where the person making that notification has continuously held qualification as an insured person under National Health Insurance since before becoming a mid- to long-term resident, etc. (meaning a mid- to long-term resident, etc. prescribed in Article 30-46 of the Act; the same applies from the following Article through Article 28)): the following matters;

イThe date on which the person acquired qualification as an insured person under National Health Insurance;

ロThe insured person's symbol and number pertaining to the person;

ハIf the householder of the household to which the person belongs is subject to the application of Article 54-3, paragraph (1) or the main text of paragraph (2) of the National Health Insurance Act, an indication to that effect.

一Moving-in notification (excluding one pertaining to changing an address within the area of a prefecture), notification under Article 30-46 of the Act, and notification under Article 30-47 of the Act (excluding the notification listed in item (iii)): the following matters;

イAn indication that the person has acquired qualification as an insured person under latter-stage elderly medical care;

ロIf there is already a person who has acquired qualification as an insured person under latter-stage elderly medical care in the household to which the person has come to belong, the insured person number of that insured person (meaning the insured person number prescribed in Article 161-2, paragraph (1) of the Act on Assurance of Medical Care for Elderly People; the same applies hereinafter in this Article);

二Notification of a move within a municipality, moving-out notification, and notification of household change: the insured person number pertaining to the person;

三Notification under Article 30-47 of the Act (limited to where the person making that notification has continuously held qualification as an insured person under latter-stage elderly medical care since before becoming a mid- to long-term resident, etc.): the following matters.

イThe date on which the person acquired qualification as an insured person under latter-stage elderly medical care;

ロThe insured person number pertaining to the person.

一Moving-in notification, notification under Article 30-46 of the Act, and notification under Article 30-47 of the Act (excluding the notification listed in item (iii)): an indication that the person holds qualification as an insured person under long-term care insurance;

二Notification of a move within a municipality, moving-out notification, and notification of household change: the insured person number (meaning the insured person number prescribed in Article 201-2, paragraph (1) of the Long-Term Care Insurance Act; the same applies in (b) of the following item);

三Notification under Article 30-47 of the Act (limited to where the person making that notification has continuously held qualification as an insured person under long-term care insurance since before becoming a mid- to long-term resident, etc.): the following matters.

イThe date on which the person became an insured person under long-term care insurance;

ロThe insured person number.

一Moving-in notification and notification under Article 30-46 of the Act: the following matters;

イFor a person who has continued to be an insured person under National Pension of the same category since the previous address, the category of insured person under National Pension and, if the person is a person prescribed in Article 22, paragraph (1), item (vii) of the Act or a person making the notification under Article 30-46, the Basic Pension Number;

ロFor a person whose category of insured person under National Pension has changed by reason of moving in, the category of insured person under National Pension after the change and, if the person is a person prescribed in Article 22, paragraph (1), item (vii) of the Act or a person making the notification under Article 30-46, the Basic Pension Number;

ハFor a person who has become an insured person under National Pension by moving in, the category of insured person under National Pension and, if the person has previously been an insured person under National Pension, the Basic Pension Number and, if the person's name changed while the person was not an insured person under National Pension, the name at the time the person last ceased to be an insured person under National Pension;

二Notification of a move within a municipality and moving-out notification: an indication that the person is an insured person under National Pension;

三Notification under Article 30-47 of the Act: the following matters.

イFor a person who has continued to be an insured person under National Pension of the same category since before becoming a mid- to long-term resident, etc., the category of insured person under National Pension and the Basic Pension Number;

ロFor a person whose category of insured person under National Pension has changed by reason of becoming a mid- to long-term resident, etc., the category of insured person under National Pension after the change and the Basic Pension Number;

ハFor a person who has become an insured person under National Pension by reason of becoming a mid- to long-term resident, etc., the category of insured person under National Pension and, if the person has previously been an insured person under National Pension, the Basic Pension Number and, if the person's name changed while the person was not an insured person under National Pension, the name at the time the person last ceased to be an insured person under National Pension.
