Article 27-3Matters to Be Additionally Stated Concerning a Person Who Is an Insured Person under Long-Term Care Insurance
第二十七条の三（介護保険の被保険者である者に係る付記事項）
The matters specified by Cabinet Order as prescribed in Article 28-3 of the Act are the matters listed in each of the following items, in accordance with the classification of notification listed in that item.
法第二十八条の三に規定する政令で定める事項は、次の各号に掲げる届出の区分に応じ、当該各号に掲げる事項とする。
Moving-in notification, notification under Article 30-46 of the Act, and notification under Article 30-47 of the Act (excluding the notification listed in item (iii)): an indication that the person holds qualification as an insured person under long-term care insurance;
Notification of a move within a municipality, moving-out notification, and notification of household change: the insured person number (meaning the insured person number prescribed in Article 201-2, paragraph (1) of the Long-Term Care Insurance Act; the same applies in (b) of the following item);
転居届、転出届及び世帯変更届 被保険者番号（介護保険法第二百一条の二第一項に規定する被保険者番号をいう。次号ロにおいて同じ。）
Notification under Article 30-47 of the Act (limited to where the person making that notification has continuously held qualification as an insured person under long-term care insurance since before becoming a mid- to long-term resident, etc.): the following matters.
The date on which the person became an insured person under long-term care insurance;
