Article 30Special Provisions for Notification Made in a Document Bearing Additional Statements
第三十条（付記がされた書面で届出をする場合の特例）
A person who is to make a notification in a document bearing the additional statements under Articles 28 through 28-3 of the Act must, if the person has been issued the document prescribed in Article 9, paragraph (2) of the National Health Insurance Act, the document prescribed in Article 54, paragraph (3) of the Act on Assurance of Medical Care for Elderly People, or a certificate of insured person under long-term care insurance (meaning the certificate of insured person set forth in Article 12, paragraph (3) of the Long-Term Care Insurance Act) pertaining to that person, make the notification with these attached.
法第二十八条から第二十八条の三までの規定による付記がされた書面で届出をすべき者は、その者に係る国民健康保険法第九条第二項に規定する書面、高齢者の医療の確保に関する法律第五十四条第三項に規定する書面又は介護保険の被保険者証（介護保険法第十二条第三項の被保険者証をいう。）の交付を受けているときは、これらを添えて、その届出をしなければならない。
A person who is to make a notification in a document bearing the additional statement under Article 29 of the Act (limited to one bearing the additional statement of the Basic Pension Number) must, if the person has been issued a document specified by Order of the Ministry of Internal Affairs and Communications that can establish the Basic Pension Number, make the notification with it attached.
