A written declaration of estimated insurance premiums, a written declaration of increased estimated insurance premiums and a written declaration of final insurance premiums in accordance with Form No. 6 of the former Labor Insurance Premium Collection Ordinance, a report on grouped businesses with a definite term in accordance with Form No. 7 of the former Labor Insurance Premium Collection Ordinance, a written claim for a refund of labor insurance premiums in accordance with Form No. 8 of the former Labor Insurance Premium Collection Ordinance, a notification of termination of entrustment of processing of labor insurance administrative affairs in accordance with Form No. 17 of the former Labor Insurance Premium Collection Ordinance, a list of employers entrusting processing of labor insurance administrative affairs in accordance with Form No. 18 of the former Labor Insurance Premium Collection Ordinance, a book of collection and payment of labor insurance premiums, etc. in accordance with Form No. 19 of the former Labor Insurance Premium Collection Ordinance, and a notification of appointment or dismissal of an agent in accordance with Form No. 23 of the former Labor Insurance Premium Collection Ordinance, that have actually been submitted at the time of enforcement of this Ministerial Order, are deemed to be, respectively, a written declaration of estimated insurance premiums, a written declaration of increased estimated insurance premiums and a written declaration of final insurance premiums in accordance with Form No. 6 of the new Labor Insurance Premium Collection Ordinance, a report on grouped businesses with a definite term in accordance with Form No. 7 of the new Labor Insurance Premium Collection Ordinance, a written claim for a refund of labor insurance premiums in accordance with Form No. 8 of the new Labor Insurance Premium Collection Ordinance, a notification of termination of entrustment of processing of labor insurance administrative affairs, etc. in accordance with Form No. 17 of the new Labor Insurance Premium Collection Ordinance, a list of employers entrusting processing of labor insurance administrative affairs, etc. in accordance with Form No. 18 of the new Labor Insurance Premium Collection Ordinance, a book of collection and payment of labor insurance premiums, etc. in accordance with Form No. 19 of the new Labor Insurance Premium Collection Ordinance, and a notification of appointment or dismissal of an agent in accordance with Form No. 23 of the new Labor Insurance Premium Collection Ordinance.
この省令の施行の際現に提出されている旧徴収則様式第六号による概算保険料申告書、増加概算保険料申告書及び確定保険料申告書、旧徴収則様式第七号による一括有期事業報告書、旧徴収則様式第八号による労働保険料還付請求書、旧徴収則様式第十七号による労働保険事務処理委託解除届、旧徴収則様式第十八号による労働保険事務処理委託事業主名簿、旧徴収則様式第十九号による労働保険料等徴収及び納付簿並びに旧徴収則様式第二十三号による代理人選任・解任届は、それぞれ、新徴収則様式第六号による概算保険料申告書、増加概算保険料申告書及び確定保険料申告書、新徴収則様式第七号による一括有期事業報告書、新徴収則様式第八号による労働保険料還付請求書、新徴収則様式第十七号による労働保険事務等処理委託解除届、新徴収則様式第十八号による労働保険事務等処理委託事業主名簿、新徴収則様式第十九号による労働保険料等徴収及び納付簿並びに新徴収則様式第二十三号による代理人選任・解任届とみなす。
