A day worker who intends to obtain the authorization under Article 43, paragraph (1), item (iv) of the Act must appear at the competent Public Employment Security Office and submit to the Chief of the competent Public Employment Security Office an application for voluntary enrollment as an insured day worker (Form No. 26), accompanied by a copy of the certificate of residence or a certificate of the matters stated in the certificate of residence.
日雇労働者は、法第四十三条第一項第四号の認可を受けようとするときは、管轄公共職業安定所に出頭し、日雇労働被保険者任意加入申請書（様式第二十六号）に住民票の写し又は住民票記載事項証明書を添えて管轄公共職業安定所の長に提出しなければならない。
The provisions of the second sentence of paragraph (1) and paragraph (2) of the preceding Article apply mutatis mutandis to the submission of the application for voluntary enrollment as an insured day worker referred to in the preceding paragraph.
