Supplementary Provisions

①This Ministerial Order comes into effect as of April 1, 2009.

②A notification of establishment of insurance relation, a notification of entrustment of processing of labor insurance administrative affairs, etc. and a written application for voluntary enrollment in accordance with Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance prior to the amendment by this Ministerial Order (hereinafter referred to as the "former Labor Insurance Premium Collection Ordinance"), a notification of change of name, location, etc. in accordance with Form No. 2 of the former Labor Insurance Premium Collection Ordinance, a written application for approval to make a subcontractor the employer in accordance with Form No. 4 of the former Labor Insurance Premium Collection Ordinance, a notification of change of name and location of a continued business subject to grouping in accordance with Form No. 5-2 of the former Labor Insurance Premium Collection Ordinance, and a report on grouped businesses with a definite term (construction business) in accordance with Form No. 7 (A) of the former Labor Insurance Premium Collection Ordinance, and a notification of entrustment of processing of labor insurance administrative affairs, etc. in accordance with Form No. 7 of the Ordinance for Enforcement of the Act on Asbestos Health Damage Relief in relation to the Ministry of Health, Labour and Welfare prior to the amendment by this Ministerial Order, that have actually been submitted at the time of enforcement of this Ministerial Order, are deemed to be, respectively, a notification of establishment of insurance relation, a notification of entrustment of processing of labor insurance administrative affairs, etc. and a written application for voluntary enrollment in accordance with Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance as amended by this Ministerial Order (hereinafter referred to as the "new Labor Insurance Premium Collection Ordinance"), a notification of change of name, location, etc. in accordance with Form No. 2 of the new Labor Insurance Premium Collection Ordinance, a written application for approval to make a subcontractor the employer in accordance with Form No. 4 of the new Labor Insurance Premium Collection Ordinance, a notification of change of name and location of a continued business subject to grouping in accordance with Form No. 5-2 of the new Labor Insurance Premium Collection Ordinance, and a report on grouped businesses with a definite term (construction business) in accordance with Form No. 7 (A) of the new Labor Insurance Premium Collection Ordinance, and a notification of entrustment of processing of labor insurance administrative affairs, etc. in accordance with Form No. 7 of the Ordinance for Enforcement of the Act on Asbestos Health Damage Relief in relation to the Ministry of Health, Labour and Welfare as amended by this Ministerial Order (hereinafter referred to as the "new Asbestos Relief Ordinance").

③A notification of establishment of insurance relation under Article 4, paragraph (2) of the new Labor Insurance Premium Collection Ordinance, a notification of change of name, location, etc. under Article 5, paragraph (2) of the new Labor Insurance Premium Collection Ordinance, a written application for approval to make a subcontractor the employer under Article 8 of the new Labor Insurance Premium Collection Ordinance, a notification of change of name and location of a continued business subject to grouping under Article 10, paragraph (4) of the new Labor Insurance Premium Collection Ordinance, a report on grouped businesses with a definite term (construction business) under Article 34 of the new Labor Insurance Premium Collection Ordinance, a notification of entrustment of processing of labor insurance administrative affairs, etc. under Article 60, paragraph (1) of the new Labor Insurance Premium Collection Ordinance, a written application for voluntary enrollment under Article 2, paragraph (1) of the Supplementary Provisions of the new Labor Insurance Premium Collection Ordinance, and a notification of entrustment of processing of labor insurance administrative affairs, etc. under Article 2-8, paragraph (1) of the new Asbestos Relief Ordinance may, for the time being, continue to be in the former forms.
