附　則

This Ministerial Order comes into effect as of April 1, 2008.

この省令は、平成二十年四月一日から施行する。

A notification of establishment of insurance relation, a notification of entrustment of processing of labor insurance administrative affairs, etc. and a written application for voluntary enrollment in accordance with Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance prior to the amendment by this Ministerial Order (hereinafter referred to as the "former Labor Insurance Premium Collection Ordinance"), a notification of change of name, location, etc. in accordance with Form No. 2 of the former Labor Insurance Premium Collection Ordinance, and a notification of entrustment of processing of labor insurance administrative affairs, etc. in accordance with Form No. 7 of the Ordinance for Enforcement of the Act on Asbestos Health Damage Relief in relation to the Ministry of Health, Labour and Welfare prior to the amendment by this Ministerial Order, that have actually been submitted at the time of enforcement of this Ministerial Order, are deemed to be, respectively, a notification of establishment of insurance relation, a notification of entrustment of processing of labor insurance administrative affairs, etc. and a written application for voluntary enrollment in accordance with Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance as amended by this Ministerial Order (hereinafter referred to as the "new Labor Insurance Premium Collection Ordinance"), a notification of change of name, location, etc. in accordance with Form No. 2 of the new Labor Insurance Premium Collection Ordinance, and a notification of entrustment of processing of labor insurance administrative affairs, etc. in accordance with Form No. 7 of the Ordinance for Enforcement of the Act on Asbestos Health Damage Relief in relation to the Ministry of Health, Labour and Welfare as amended by this Ministerial Order (hereinafter referred to as the "new Asbestos Relief Ordinance").

この省令の施行の際現に提出されているこの省令による改正前の労働保険の保険料の徴収等に関する法律施行規則（以下「旧徴収則」という。）様式第一号による保険関係成立届、労働保険事務等処理委託届及び任意加入申請書並びに旧徴収則様式第二号による名称、所在地等変更届並びにこの省令による改正前の厚生労働省関係石綿による健康被害の救済に関する法律施行規則様式第七号による労働保険事務等処理委託届は、それぞれ、この省令による改正後の労働保険の保険料の徴収等に関する法律施行規則（以下「新徴収則」という。）様式第一号による保険関係成立届、労働保険事務等処理委託届及び任意加入申請書並びに新徴収則様式第二号による名称、所在地等変更届並びにこの省令による改正後の厚生労働省関係石綿による健康被害の救済に関する法律施行規則（以下「新石綿則」という。）様式第七号による労働保険事務等処理委託届とみなす。

A notification of establishment of insurance relation under Article 4, paragraph (2) of the new Labor Insurance Premium Collection Ordinance, a notification of change of name, location, etc. under Article 5, paragraph (2) of the new Labor Insurance Premium Collection Ordinance, a notification of entrustment of processing of labor insurance administrative affairs, etc. under Article 60, paragraph (1) of the new Labor Insurance Premium Collection Ordinance, a written application for voluntary enrollment under Article 2, paragraph (1) of the Supplementary Provisions of the new Labor Insurance Premium Collection Ordinance, and a notification of entrustment of processing of labor insurance administrative affairs, etc. under Article 2-8, paragraph (1) of the new Asbestos Relief Ordinance may, for the time being, continue to be in the former forms.

新徴収則第四条第二項の規定による保険関係成立届、新徴収則第五条第二項の規定による名称、所在地等変更届、新徴収則第六十条第一項の規定による労働保険事務等処理委託届、新徴収則附則第二条第一項の規定による任意加入申請書及び新石綿則第二条の八第一項の規定による労働保険事務等処理委託届は、当分の間、なお従前の様式によることができる。
