第一条（施行期日）
This Ministerial Order comes into effect as of October 26, 1998.
この省令は、平成十年十月二十六日から施行する。

第二条（経過措置）
A notification of establishment of insurance relation in accordance with Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance prior to the amendment by this Ministerial Order (hereinafter referred to as the "Former Rules"), a notification of change of name, location, etc. in accordance with Form No. 2 of the Former Rules, a notification of commencement of grouped businesses with a definite term in accordance with Form No. 3 of the Former Rules, a written application for approval to make a subcontractor the employer in accordance with Form No. 4 of the Former Rules, a written application for grouping of continued businesses and a notification of change of name and location of a continued business subject to grouping in accordance with Form No. 5 of the Former Rules, a written declaration for application of the special provisions for the industrial accident insurance rate in accordance with Form No. 5-2 of the Former Rules, a written declaration of estimated insurance premiums, a written declaration of increased estimated insurance premiums and a written declaration of final insurance premiums in accordance with Form No. 6 of the Former Rules, a report on grouped businesses with a definite term in accordance with Form No. 7 of the Former Rules, a written claim for a refund of labor insurance premiums in accordance with Form No. 8 of the Former Rules, a written application for issuance of an employment insurance stamp purchase book and a written application for renewal of an employment insurance stamp purchase book in accordance with Form No. 9 of the Former Rules, a written application for designation of a stamp insurance premium payment register in accordance with Form No. 11 of the Former Rules, a written application for approval of installation of a stamp insurance premium payment register in accordance with Form No. 12 of the Former Rules, a written application for issuance of a starting ticket receipt book in accordance with Form No. 13 of the Former Rules, a report on the status of payment of stamp insurance premiums and a report on the status of use of the stamp insurance premium payment register in accordance with Form No. 15 of the Former Rules, a written application for approval as a labor insurance administrative affairs association in accordance with Form No. 16 of the Former Rules, a notification of entrustment, etc. of processing of labor insurance administrative affairs in accordance with No. 17 of the Former Rules, a notification of appointment or dismissal of an agent in accordance with Form No. 23 of the Former Rules, a written application for voluntary enrollment in accordance with Form No. 26 of the Former Rules, and a written application for extinction of insurance relation in accordance with Form No. 27 of the Former Rules, that have actually been submitted at the time of enforcement of this Ministerial Order, are deemed to be, respectively, a notification of establishment of insurance relation in accordance with Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance as amended by this Ministerial Order (hereinafter referred to as the "New Rules"), a notification of change of name, location, etc. in accordance with Form No. 2 of the New Rules, a notification of commencement of grouped businesses with a definite term in accordance with Form No. 3 of the New Rules, a written application for approval to make a subcontractor the employer in accordance with Form No. 4 of the New Rules, a written application for grouping of continued businesses in accordance with Form No. 5 of the New Rules and a notification of change of name and location of a continued business subject to grouping in accordance with Form No. 5-2 of the New Rules, a written declaration for application of the special provisions for the industrial accident insurance rate in accordance with Form No. 5-3 of the New Rules, a written declaration of estimated insurance premiums, a written declaration of increased estimated insurance premiums and a written declaration of final insurance premiums in accordance with Form No. 6 of the New Rules, a report on grouped businesses with a definite term in accordance with Form No. 7 of the New Rules, a written claim for a refund of labor insurance premiums in accordance with Form No. 8 of the Former Rules, a written application for issuance of an employment insurance stamp purchase book and a written application for renewal of an employment insurance stamp purchase book in accordance with Form No. 9 of the New Rules, a written application for designation of a stamp insurance premium payment register in accordance with Form No. 11 of the New Rules, a written application for approval of installation of a stamp insurance premium payment register in accordance with Form No. 12 of the New Rules, a written application for issuance of a starting ticket receipt book in accordance with Form No. 13 of the New Rules, a report on the status of payment of stamp insurance premiums and a report on the status of use of the stamp insurance premium payment register in accordance with Form No. 15 of the New Rules, a written application for approval as a labor insurance administrative affairs association in accordance with Form No. 16 of the New Rules, a notification of entrustment of processing of labor insurance administrative affairs in accordance with Form No. 1 of the New Rules or a notification of termination of entrustment of processing of labor insurance administrative affairs in accordance with Form No. 17 of the New Rules, a notification of appointment or dismissal of an agent in accordance with Form No. 23 of the New Rules, a written application for voluntary enrollment in accordance with Form No. 1 of the New Rules, and a written application for extinction of insurance relation in accordance with Form No. 27 of the New Rules.
この省令の施行の際現に提出されているこの省令による改正前の労働保険の保険料の徴収等に関する法律施行規則（以下「旧規則」という。）様式第一号による保険関係成立届、旧規則様式第二号による名称、所在地等変更届、旧規則様式第三号による一括有期事業開始届、旧規則様式第四号による下請負人を事業主とする認可申請書、旧規則様式第五号による継続事業一括申請書及び継続被一括事業名称・所在地変更届、旧規則様式第五号の二による労災保険率特例適用申告書、旧規則様式第六号による概算保険料申告書、増加概算保険料申告書及び確定保険料申告書、旧規則様式第七号による一括有期事業報告書、旧規則様式第八号による労働保険料還付請求書、旧規則様式第九号による雇用保険印紙購入通帳交付申請書及び雇用保険印紙購入通帳更新申請書、旧規則様式第十一号による印紙保険料納付計器指定申請書、旧規則様式第十二号による印紙保険料納付計器設置承認申請書、旧規則様式第十三号による始動票札受領通帳交付申請書、旧規則様式第十五号による印紙保険料納付状況報告書及び印紙保険料納付計器使用状況報告書、旧規則様式第十六号による労働保険事務組合認可申請書、旧規則第十七号による労働保険事務処理委託等届、旧規則様式第二十三号による代理人選任・解任届、旧規則様式第二十六号による任意加入申請書並びに旧規則様式第二十七号による保険関係消滅申請書は、それぞれ、この省令による改正後の労働保険の保険料の徴収等に関する法律施行規則（以下「新規則」という。）様式第一号による保険関係成立届、新規則様式第二号による名称、所在地等変更届、新規則様式第三号による一括有期事業開始届、新規則様式第四号による下請負人を事業主とする認可申請書、新規則様式第五号による継続事業一括申請書及び新規則様式第五号の二による継続被一括事業名称・所在地変更届、新規則様式第五号の三による労災保険率特例適用申告書、新規則様式第六号による概算保険料申告書、増加概算保険料申告書及び確定保険料申告書、新規則様式第七号による一括有期事業報告書、旧規則様式第八号による労働保険料還付請求書、新規則様式第九号による雇用保険印紙購入通帳交付申請書及び雇用保険印紙購入通帳更新申請書、新規則様式第十一号による印紙保険料納付計器指定申請書、新規則様式第十二号による印紙保険料納付計器設置承認申請書、新規則様式第十三号による始動票札受領通帳交付申請書、新規則様式第十五号による印紙保険料納付状況報告書及び印紙保険料納付計器使用状況報告書、新規則様式第十六号による労働保険事務組合認可申請書、新規則様式第一号による労働保険事務処理委託届又は新規則様式第十七号による労働保険事務処理委託解除届、新規則様式第二十三号による代理人選任・解任届、新規則様式第一号による任意加入申請書並びに新規則様式第二十七号による保険関係消滅申請書とみなす。
