第四条
With regard to the written declaration pertaining to labor insurance premiums for the effective year of the amending Order, Form No. 6 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance may be used after making necessary revisions.
改正省令施行年度の労働保険料に係る申告書については、労働保険の保険料の徴収等に関する法律施行規則様式第六号に必要な改定をして使用することができる。
