Supplementary Provisions, Article 3Application for Extinction of Insurance Relation for a Temporary Voluntary Applicable Business
第三条（暫定任意適用事業についての保険関係消滅の申請）
An employer who intends to apply for extinction of the insurance relation pertaining to employment insurance pursuant to the provisions of Article 4, paragraph (1) of the Supplementary Provisions of the Act must submit to the Director of the Prefectural Labor Bureau Concerned a written application stating the following matters:
法附則第四条第一項の規定により、雇用保険に係る保険関係の消滅の申請をしようとする事業主は、次に掲げる事項を記載した申請書を所轄都道府県労働局長に提出しなければならない。
the labor insurance number;
労働保険番号
in the case of a business for which an insurance relation pertaining to employment insurance has been established, the employment insurance applicable establishment number;
雇用保険に係る保険関係が成立している事業にあつては、雇用保険適用事業所番号
the employer's name or title, and address or location;
事業主の氏名又は名称及び住所又は所在地
the name of the business, the place where the business is carried out, the date on which the insurance relation was established, an outline of the business, the number of workers in the business, the type of business, and the wage closing date;
事業の名称、事業の行われる場所、保険関係成立の年月日、事業の概要、事業に係る労働者数、事業の種類及び賃金締切日
whether or not there is a recipient of the insurance benefits prescribed in Article 7 of the Industrial Accident Insurance Act;
労災保険法第七条に規定する保険給付の受給者の有無
the reason for the application.
申請の理由
A document that can certify that the consent of the workers prescribed in Article 4, paragraph (2) of the Supplementary Provisions of the Act has been obtained must be attached to the written application under the preceding paragraph.
