A notification of commencement of grouped businesses with a definite term under Article 6, paragraph (3) of the new Labor Insurance Premium Collection Ordinance, a written application for approval to make a subcontractor the employer under Article 8 of the new Labor Insurance Premium Collection Ordinance, a report on grouped businesses with a definite term under Article 34 of the new Labor Insurance Premium Collection Ordinance, a written claim for a refund of labor insurance premiums under Article 36, paragraph (2) of the new Labor Insurance Premium Collection Ordinance, a written application for issuance of an employment insurance stamp purchase book under Article 42, paragraph (1) of the new Labor Insurance Premium Collection Ordinance, a written application for designation of a stamp insurance premium payment register under Article 45, paragraph (1) of the new Labor Insurance Premium Collection Ordinance, a written application for approval of installation of a stamp insurance premium payment register under Article 47, paragraph (1) of the new Labor Insurance Premium Collection Ordinance, a written application for issuance of a starting ticket receipt book and the starting ticket receipt book under Article 50, paragraph (1) of the new Labor Insurance Premium Collection Ordinance, a written application for approval as a labor insurance administrative affairs association under Article 59, paragraph (1) of the new Labor Insurance Premium Collection Ordinance, a book of collection and payment of labor insurance premiums, etc. under Article 64, item (ii) of the new Labor Insurance Premium Collection Ordinance, a notification of appointment or dismissal of an agent under Article 71, paragraph (2) of the new Labor Insurance Premium Collection Ordinance, and a written application for extinction of insurance relation under Article 3, paragraph (1) of the Supplementary Provisions of the new Labor Insurance Premium Collection Ordinance may, for the time being, continue to be in the former forms.

新徴収則第六条第三項の規定による一括有期事業開始届、新徴収則第八条の規定による下請負人を事業主とする認可申請書、新徴収則第三十四条の規定による一括有期事業報告書、新徴収則第三十六条第二項の規定による労働保険料還付請求書、新徴収則第四十二条第一項の規定による雇用保険印紙購入通帳交付申請書、新徴収則第四十五条第一項の規定による印紙保険料納付計器指定申請書、新徴収則第四十七条第一項の規定による印紙保険料納付計器設置承認申請書、新徴収則第五十条第一項の規定による始動票札受領通帳交付申請書及び始動票札受領通帳、新徴収則第五十九条第一項の規定による労働保険事務組合認可申請書、新徴収則第六十四条第二号の規定による労働保険料等徴収及び納付簿、新徴収則第七十一条第二項の規定による代理人選任・解任届並びに新徴収則附則第三条第一項の規定による保険関係消滅申請書は、当分の間、なお従前の様式によることができる。
