Supplementary Provisions, Article 2Transitional Measures

①A notification of establishment of insurance relation in accordance with Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance prior to the amendment by this Ministerial Order (hereinafter referred to as the "Former Rules"), a notification of change of name, location, etc. in accordance with Form No. 2 of the Former Rules, a notification of commencement of grouped businesses with a definite term in accordance with Form No. 3 of the Former Rules, a written application for approval to make a subcontractor the employer in accordance with Form No. 4 of the Former Rules, a written application for grouping of continued businesses in accordance with Form No. 5 of the Former Rules, a written declaration of estimated insurance premiums, a written declaration of increased estimated insurance premiums and a written declaration of final insurance premiums in accordance with Form No. 6 of the Former Rules, a report on grouped businesses with a definite term in accordance with Form No. 7 of the Former Rules, a written claim for a refund of labor insurance premiums in accordance with Form No. 8 of the Former Rules, a report on the status of payment of stamp insurance premiums in accordance with Form No. 15 of the Former Rules, a written application for approval as a labor insurance administrative affairs association in accordance with Form No. 16 of the Former Rules, a notification of entrustment, etc. of processing of labor insurance administrative affairs in accordance with Form No. 17 of the Former Rules, a notification of appointment or dismissal of an agent in accordance with Form No. 23 of the Former Rules, a written application for voluntary enrollment in accordance with Form No. 26 of the Former Rules, and a written application for extinction of insurance relation in accordance with Form No. 27 of the Former Rules, that have actually been submitted at the time of enforcement of this Ministerial Order, are deemed to be, respectively, a notification of establishment of insurance relation in accordance with Form No. 1 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance as amended by this Ministerial Order (hereinafter referred to as the "New Rules"), a notification of change of name, location, etc. in accordance with Form No. 2 of the New Rules, a notification of commencement of grouped businesses with a definite term in accordance with Form No. 3 of the New Rules, a written application for approval to make a subcontractor the employer in accordance with Form No. 4 of the New Rules, a written application for grouping of continued businesses in accordance with Form No. 5 of the New Rules, a written declaration of estimated insurance premiums, a written declaration of increased estimated insurance premiums and a written declaration of final insurance premiums in accordance with Form No. 6 of the New Rules, a report on grouped businesses with a definite term in accordance with Form No. 7 of the New Rules, a written claim for a refund of labor insurance premiums in accordance with Form No. 8 of the New Rules, a report on the status of payment of stamp insurance premiums in accordance with Form No. 15 of the New Rules, a written application for approval as a labor insurance administrative affairs association in accordance with Form No. 16 of the New Rules, a notification of entrustment, etc. of processing of labor insurance administrative affairs in accordance with Form No. 17 of the New Rules, a notification of appointment or dismissal of an agent in accordance with Form No. 23 of the New Rules, a written application for voluntary enrollment in accordance with Form No. 26 of the New Rules, and a written application for extinction of insurance relation in accordance with Form No. 27 of the New Rules.

②A notification of establishment of insurance relation under Article 4, paragraph (2) of the New Rules, a notification of change of name, location, etc. under Article 5, paragraph (2) of the New Rules, a notification of commencement of grouped businesses with a definite term under Article 6, paragraph (3) of the New Rules, a written application for approval to make a subcontractor the employer under Article 8 of the New Rules, a written declaration of estimated insurance premiums under Article 24, paragraph (3) of the New Rules, a written declaration of increased estimated insurance premiums under Article 25, paragraph (3) of the New Rules, a written declaration of final insurance premiums under Article 33, paragraph (2) of the New Rules, a report on grouped businesses with a definite term under Article 34 of the New Rules, a written claim for a refund of labor insurance premiums under Article 36, paragraph (2) of the New Rules, a written application for issuance of an employment insurance stamp purchase book under Article 42 of the New Rules, a written application for approval as a labor insurance administrative affairs association under Article 59, paragraph (1) of the New Rules, a notification of entrustment, etc. of processing of labor insurance administrative affairs under Article 60 of the New Rules, a list of employers entrusting processing of labor insurance administrative affairs under Article 64, item (i) of the New Rules, a register of processing of notification affairs relating to insured persons of employment insurance, etc. under Article 64, item (iii) of the New Rules, a notification of appointment or dismissal of an agent under Article 71, paragraph (2) of the New Rules, a written application for voluntary enrollment under Article 2, paragraph (1) of the Supplementary Provisions of the New Rules, and a written application for extinction of insurance relation under Article 3, paragraph (1) of the Supplementary Provisions of the New Rules may, for the time being, continue to be in the former forms.
