Supplementary Provisions, Article 2Application for Voluntary Enrollment in Employment Insurance

第二条（雇用保険の任意加入の申請）

An employer who intends to apply for enrollment in employment insurance pursuant to the provisions of Article 2, paragraph (1) of the Supplementary Provisions of the Act must submit to the Director of the Prefectural Labor Bureau Concerned a written application stating the following matters:

法附則第二条第一項の規定により、雇用保険の加入の申請をしようとする事業主は、次に掲げる事項を記載した申請書を所轄都道府県労働局長に提出しなければならない。

the employer's name or title, and address or location;

事業主の氏名又は名称及び住所又は所在地

the name of the business, the place where the business is carried out, an outline of the business, the type of business, and the number of workers in the business;

事業の名称、事業の行われる場所、事業の概要、事業の種類及び事業に係る労働者数

in the case of a business with a definite term, the scheduled period of the business;

有期事業にあつては、事業の予定される期間

where the employer has a corporate number, the employer's corporate number.

事業主が法人番号を有する場合には、当該事業主の法人番号

A document that can certify that the consent of the workers prescribed in Article 2, paragraph (2) of the Supplementary Provisions of the Act has been obtained must be attached to the written application under the preceding paragraph.

前項の申請書には、法附則第二条第二項に規定する労働者の同意を得たことを証明することができる書類を添えなければならない。
