Article 5Matters to Be Stated in a Certificate of Residence Concerning Qualification as an Insured Person under National Pension
第五条（国民年金の被保険者の資格に関する住民票の記載事項）
The matters concerning qualification as an insured person under National Pension prescribed in Article 7, item (xi) of the Act that are specified by Cabinet Order are the following matters:
法第七条第十一号に規定する国民年金の被保険者の資格に関する事項で政令で定めるものは、次に掲げる事項とする。
The date on which the person became, or ceased to be, an insured person under National Pension;
国民年金の被保険者となり、又は国民年金の被保険者でなくなつた年月日
The category of insured person under National Pension (meaning the distinction as to whether the person is a Category I Insured Person prescribed in Article 7, paragraph (1), item (i) of the National Pension Act or an insured person under National Pension pursuant to the provisions of laws and regulations prescribed in the preceding Article; the same applies hereinafter) and the date on which it was changed; and
国民年金の被保険者の種別（国民年金法第七条第一項第一号に規定する第一号被保険者又は前条に規定する法令の規定による国民年金の被保険者のいずれであるかの区別をいう。以下同じ。）及びその変更があつた年月日
The Basic Pension Number (meaning the Basic Pension Number prescribed in Article 14 of the National Pension Act; the same applies hereinafter).
基礎年金番号（国民年金法第十四条に規定する基礎年金番号をいう。以下同じ。）
