Article 3-2Matters to Be Stated in a Certificate of Residence Concerning Qualification as an Insured Person under Latter-Stage Elderly Medical Care
第三条の二（後期高齢者医療の被保険者の資格に関する住民票の記載事項）
The matters concerning qualification as an insured person under latter-stage elderly medical care prescribed in Article 7, item (x)-2 of the Act that are specified by Cabinet Order are the date on which the person acquired or lost that qualification.
法第七条第十号の二に規定する後期高齢者医療の被保険者の資格に関する事項で政令で定めるものは、その資格を取得し、又は喪失した年月日とする。
