Article 3Matters to Be Stated in a Certificate of Residence Concerning Qualification as an Insured Person under National Health Insurance
第三条（国民健康保険の被保険者の資格に関する住民票の記載事項）
The matters concerning qualification as an insured person under National Health Insurance prescribed in Article 7, item (x) of the Act that are specified by Cabinet Order are the date on which the person acquired or lost that qualification.
法第七条第十号に規定する国民健康保険の被保険者の資格に関する事項で政令で定めるものは、その資格を取得し、又は喪失した年月日とする。
