A notification under Chapter IV or Chapter IV-4 of the Act must be made in a document stating the address of the person actually handling the notification and the date of the notification, and signed by the person handling the notification.
法第四章又は第四章の四の規定による届出は、現に届出の任に当たつている者の住所及び届出の年月日が記載され、並びに当該届出の任に当たつている者が署名した書面でしなければならない。
