Article 17Matters Specified by Cabinet Order as Prescribed in Article 15-4, Paragraphs (2) and (3) of the Act
第十七条（法第十五条の四第二項及び第三項に規定する政令で定める事項）
The matters specified by Cabinet Order prescribed in Article 15-4, paragraphs (2) and (3) of the Act are, according to the categories of removed certificates of residence relating to the request referred to in paragraph (2) of that Article or the application referred to in paragraph (3) or paragraph (4) of that Article that are set out in the following items, the matters specified respectively in those items.
法第十五条の四第二項及び第三項に規定する政令で定める事項は、次の各号に掲げる同条第二項の請求又は同条第三項若しくは第四項の申出に係る除票の区分に応じ、当該各号に定める事項とする。
Deleted certificate of residence: the grounds for deleting the certificate of residence relating to that deleted certificate of residence (in the case of moving out, a statement that it was deleted due to moving out, the address of the destination of the move and, if the deleted certificate of residence states that the person has moved out, a statement that the person has moved out) and the date on which those grounds arose (in the case where the certificate of residence was deleted based on a moving-out notification, the scheduled date of moving out);
消除した住民票 当該消除した住民票に係る住民票を消除した事由（転出の場合にあつては、転出により消除した旨、転出先の住所及び当該消除した住民票に転出をした旨の記載がされているときは転出をした旨）及びその事由の生じた年月日（転出届に基づき住民票を消除した場合にあつては、転出の予定年月日）
Certificate of residence prior to re-preparation: a statement that the certificate of residence relating to that certificate of residence prior to re-preparation was re-prepared, and the date thereof.
改製前の住民票 当該改製前の住民票に係る住民票を改製した旨及びその年月日
