第四条
With regard to a Municipality designated pursuant to the provisions of Article 68-2, paragraph (1) of the Pre-amendment National Health Insurance Act for each fiscal year from fiscal year 2008 through fiscal year 2010, the provisions of paragraphs (3) through (6) of that Article, Article 70, paragraphs (3) through (5), Article 72-4, Article 118 and Article 9, paragraph (1) of the Supplementary Provisions of the Pre-amendment National Health Insurance Act (including the provisions of orders based on these provisions) remain in force. In this case, the phrase "the ratio of the total number of those pre-elderly insured persons to the total number of insured persons of all Municipalities" in Article 70, paragraph (5), item (ii) of the Pre-amendment National Health Insurance Act is deemed to be replaced with "the ratio of the total number of subscribers who are pre-elderly persons prescribed in Article 32, paragraph (1) of the Act on Assurance of Medical Care for Elderly People to the total number of subscribers prescribed in Article 7, paragraph (3) of that Act pertaining to all insurers (meaning the insurers prescribed in paragraph (2) of that Article)".
平成二十年度から平成二十二年度までの各年度につき改正前国保法第六十八条の二第一項の規定により指定を受けた市町村については、同条第三項から第六項まで並びに改正前国保法第七十条第三項から第五項まで、第七十二条の四、第百十八条及び附則第九条第一項の規定（これらの規定に基づく命令の規定を含む。）は、なおその効力を有する。この場合において、改正前国保法第七十条第五項第二号中「すべての市町村の被保険者の総数に対する当該前期高齢被保険者の総数の割合」とあるのは、「すべての保険者（高齢者の医療の確保に関する法律第七条第二項に規定する保険者をいう。）に係る同条第三項に規定する加入者の総数に対する同法第三十二条第一項に規定する前期高齢者である加入者の総数の割合」とする。
With regard to the application of the provisions of the preceding paragraph to a Municipality that was designated pursuant to the provisions of Article 68-2, paragraph (1) of the Pre-amendment National Health Insurance Act for fiscal year 2010 and that, in fiscal year 2012, falls under the Municipalities prescribed in Article 70, paragraph (3) of the Pre-amendment National Health Insurance Act, which is to remain in force pursuant to the provisions of the preceding paragraph, the phrase "Article 70, paragraph (5), item (ii)" in the second sentence of that paragraph is deemed to be replaced with "Article 70, paragraph (3), the phrase 'the preceding two paragraphs' is deemed to be replaced with 'Article 3, paragraphs (1) and (2) of the Supplementary Provisions of the Act Partially Amending the National Health Insurance Act (Act No. 28 of 2012)', the phrase '34/100' with '32/100', and paragraph (5), item (ii) of that Article".
平成二十二年度につき改正前国保法第六十八条の二第一項の規定により指定を受けた市町村であって平成二十四年度において前項の規定によりなおその効力を有するものとされた改正前国保法第七十条第三項に規定する市町村に該当するものに対する前項の規定の適用については、同項後段中「第七十条第五項第二号」とあるのは、「第七十条第三項中「前二項」とあるのは「国民健康保険法の一部を改正する法律（平成二十四年法律第二十八号）附則第三条第一項及び第二項」と、「百分の三十四」とあるのは「百分の三十二」と、同条第五項第二号」とする。
