A designated motor vehicle maintenance business operator must keep a designated maintenance record book and enter the following matters in it with respect to a motor vehicle for which it has issued a safety standards conformity certificate, a safety standards conformity mark or a limited safety standards conformity certificate:
指定自動車整備事業者は、指定整備記録簿を備え、保安基準適合証、保安基準適合標章又は限定保安基準適合証を交付した自動車について、次に掲げる事項を記載しなければならない。
the vehicle name and model, the chassis number and the motor model, together with the motor vehicle registration number in the case of a registered motor vehicle and the vehicle number in the case of a motor vehicle for which a vehicle number has been designated under the latter part of Article 60, paragraph (1);
an outline of the inspection and maintenance and of the examination;

the date of the examination;
検査の年月日
the name of the motor vehicle inspector;
自動車検査員の氏名
matters concerning the safety standards conformity certificate, safety standards conformity mark and limited safety standards conformity certificate that are prescribed by Order of the Ministry of Land, Infrastructure, Transport and Tourism;
国土交通省令で定める保安基準適合証、保安基準適合標章及び限定保安基準適合証に関する事項
the name or title and address of the client.

A designated maintenance record book must be preserved for two years from the date of the entry.
指定整備記録簿は、その記載の日から二年間保存しなければならない。
