With regard to the application of Article 30-14, paragraph (1) of the Order for the Enforcement of the Residential Basic Book Act (including where it is applied by replacing terms under Article 6 of the Supplementary Provisions or paragraph (1) of the preceding Article; the same applies hereinafter in this paragraph) to a person whose surname has changed for whom, on the day on which the former surname that had been entered in the certificate of residence was last deleted, no kana reading of the former surname was entered in that certificate of residence, the phrase "an entry of the former surname and the kana reading of the former surname has been made" in Article 30-14, paragraph (1) of that Order is deemed to be replaced with "an entry of the former surname has been made", and the phrase "the former surname and the kana reading of the former surname" is deemed to be replaced with "the former surname".
In the case where a removed certificate of residence pertaining to a person whose surname has changed has a former surname entered (or, for a removed certificate of residence prepared by means of a magnetic disk under Article 15-2, paragraph (2) of the Act, recorded; the same applies hereinafter in this paragraph) but no kana reading of the former surname entered, with regard to the application of Article 30-14, paragraph (9) of the Order for the Enforcement of the Residential Basic Book Act, the phrase "of the former surname and the kana reading of the former surname" in Article 30-14, paragraph (9) of that Order is deemed to be replaced with "of the former surname"; the phrase "the former surname and the kana reading of the former surname (meaning the kana reading of the former surname prescribed in Article 30-13 of the Order for the Enforcement of the Residential Basic Book Act; the same applies hereinafter in this Chapter)" in the row for Article 12, paragraph (5) as applied mutatis mutandis in Article 15-4, paragraph (5) in the table of that paragraph is deemed to be replaced with "the former surname"; the phrase "the former surname and the kana reading of the former surname" in the row for Article 12-2, paragraph (4) as applied mutatis mutandis in Article 15-4, paragraph (5) in that table is deemed to be replaced with "the former surname"; and the phrase "and the kana reading of the former surname (meaning the kana reading of the former surname prescribed in Article 30-13 of that Order), and" in the row for Article 15-4, paragraph (3) in that table is deemed to be replaced with "and".
氏に変更があった者に係る除票に旧氏の記載（法第十五条の二第二項の規定により磁気ディスクをもって調製する除票にあっては、記録。以下この項において同じ。）がされ、旧氏の振り仮名の記載がされていない場合における住民基本台帳法施行令第三十条の十四第九項の規定の適用については、同項中「旧氏及び旧氏の振り仮名の」とあるのは「旧氏の」と、同項の表第十五条の四第五項において準用する第十二条第五項の項中「旧氏及び旧氏の振り仮名（住民基本台帳法施行令第三十条の十三に規定する旧氏の振り仮名をいう。以下この章において同じ。）」とあるのは「旧氏」と、同表第十五条の四第五項において準用する第十二条の二第四項の項中「旧氏及び旧氏の振り仮名」とあるのは「旧氏」と、同表第十五条の四第三項の項中「及び旧氏の振り仮名（同令第三十条の十三に規定する旧氏の振り仮名をいう。）並びに」とあるのは「並びに」とする。
