第三十条の三（住民票コードの記載の変更請求書の提出方法）
A person who intends to request a change of the entry of a resident record code under Article 30-4, paragraph (1) of the Act must present an Individual Number Card or a document specified by Order of the Ministry of Internal Affairs and Communications when submitting the written request for change prescribed in paragraph (2) of that Article.
法第三十条の四第一項の規定により住民票コードの記載の変更の請求をしようとする者は、同条第二項に規定する変更請求書を提出する際に、個人番号カード又は総務省令で定める書類を提示しなければならない。
