Article 3-3Matters to Be Stated in a Certificate of Residence Concerning Qualification as an Insured Person under Long-Term Care Insurance
第三条の三（介護保険の被保険者の資格に関する住民票の記載事項）
The matters concerning qualification as an insured person under Long-Term Care Insurance prescribed in Article 7, item (x)-3 of the Act that are specified by Cabinet Order are the date on which the person became, or ceased to be, an insured person under Long-Term Care Insurance.
法第七条第十号の三に規定する介護保険の被保険者の資格に関する事項で政令で定めるものは、介護保険の被保険者となり、又は介護保険の被保険者でなくなつた年月日とする。
