Supplementary Provisions, Article 6Transitional Measures Accompanying the Partial Amendment of the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures
第六条（行政手続における特定の個人を識別するための番号の利用等に関する法律の一部改正に伴う経過措置）
With regard to the notification of the loss of a notification card under Article 7, paragraph (6) of the former Number Use Act and the return of the notification card under paragraph (7) of that Article by a person who, on the date on which the provisions set forth in Article 1, item (vi) of the Supplementary Provisions come into effect (referred to as the "item (vi) effective date" in the following paragraph), has actually been issued a notification card (meaning a notification card prescribed in Article 7, paragraph (1) of the former Number Use Act; hereinafter the same applies in this Article) pursuant to the provisions of Article 7, paragraph (1) or (2) of the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures prior to the amendment by the provisions of Article 4 (hereinafter referred to as the "former Number Use Act" in this paragraph and paragraph (3)) or Article 3, paragraphs (1) through (3) of the Supplementary Provisions of the former Number Use Act (referred to as a "notification card holder" in the following paragraph and paragraph (3)), the provisions then in force continue to govern.
With regard to the measures for confirming that a notification card holder is the person themselves when a Person in Charge of a Process Using an Individual Number, etc. prescribed in Article 12 of the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures receives, pursuant to the provisions of Article 14, paragraph (1) of that Act, the provision of an Individual Number prescribed in Article 2, paragraph (5) of that Act from a notification card holder (excluding a person for whom, on or after the item (vi) effective date, there has been a change in the matters stated on the notification card pertaining to that notification card holder) who is the person themselves (meaning the person themselves prescribed in Article 2, paragraph (6) of that Act; hereinafter the same applies in this paragraph), the provisions then in force continue to govern, notwithstanding the provisions of Article 16 of that Act as amended by the provisions of Article 4 (referred to as the "new Number Use Act" in the following paragraph).
番号利用法第十二条に規定する個人番号利用事務等実施者が番号利用法第十四条第一項の規定により通知カード所持者（第六号施行日以後当該通知カード所持者に係る通知カードに係る記載事項に変更があった者を除く。）である本人（番号利用法第二条第六項に規定する本人をいう。以下この項において同じ。）から番号利用法第二条第五項に規定する個人番号の提供を受けるときにおける当該通知カード所持者が本人であることを確認するための措置については、第四条の規定による改正後の番号利用法（次項において「新番号利用法」という。）第十六条の規定にかかわらず、なお従前の例による。
When the mayor of a municipality issues to a notification card holder (excluding a person who has made the notification of the loss of the notification card under Article 7, paragraph (6) of the former Number Use Act and the return of the notification card under paragraph (7) of that Article, as to which the provisions then in force continue to govern pursuant to the provisions of paragraph (1)) an Individual Number Card pertaining to that person (meaning an Individual Number Card prescribed in Article 2, paragraph (7) of the new Number Use Act), the mayor of the municipality must, in addition to taking the measures prescribed in Article 17, paragraph (1) of the new Number Use Act, receive the return of the notification card from that person.
