第二条（様式に関する経過措置）
For the written declaration pertaining to the labor insurance premiums for the insurance year beginning on April 1, 1981, Form No. 6 of the Ordinance for Enforcement of the Act on the Collection, etc. of Insurance Premiums of Labor Insurance may be used with the necessary revisions.
昭和五十六年四月一日から始まる保険年度の労働保険料に係る申告書については、労働保険の保険料の徴収等に関する法律施行規則様式第六号に必要な改定をして使用することができる。
