A person who intends to receive certification of a motor vehicle specified maintenance business must submit to the Director of a District Transport Bureau a written application stating the following matters:
自動車特定整備事業の認証を受けようとする者は、次に掲げる事項を記載した申請書を地方運輸局長に提出しなければならない。
name or title and address and, in the case of a juridical person, the names of its officers;
氏名又は名称及び住所並びに法人にあつては、その役員の氏名
the type of motor vehicle specified maintenance business;
自動車特定整備事業の種類
the location of the place of business;
事業場の所在地
in the case of a person who intends to receive certification limiting the scope of operations pursuant to paragraph (2) of the preceding Article, the types of motor vehicles covered and other scope of operations.
A document certifying that the application conforms to the requirements set out in the items of paragraph (1) of the following Article must be attached to the written application under the preceding paragraph.
The Director of a District Transport Bureau may request a person who has applied for certification of a motor vehicle specified maintenance business to submit, in addition to what is provided for in the preceding two paragraphs, a certificate of registered matters of that person and other necessary documents.
