A person aged 65 or over who has a domicile in the municipality's area (Article 9, item (i)). Residents of certain facilities for persons with disabilities, and others a Ministry of Health, Labour and Welfare Order names, are kept out of the insurance for the time being by Article 11, paragraph (1) of the Act for Enforcement of the Long-Term Care Insurance Act.

A dependent spouse (被扶養配偶者) of a category 2 insured person (第二号被保険者) — one who mainly lives on that person's income and lives in Japan or is treated as based there — aged 20 to 59 (Article 7, paragraph (1), item (iii)).

A person living in Japan aged 20 to 59 who is neither a category 2 insured person (第二号被保険者) nor a category 3 insured person (第三号被保険者), other than one who can receive old-age benefits under the Employees' Pension Insurance Act and similar benefits (厚生年金保険法に基づく老齢給付等) or whom a Ministry of Health, Labour and Welfare Order (厚生労働省令) excludes (Article 7, paragraph (1), item (i)).
