A person who intends to obtain a license referred to in the preceding Article must, pursuant to the provisions of Cabinet Office Order, submit a written application for a license containing the following particulars to the competent minister:
trade name or other name and address;
商号又は名称及び住所
amount of capital or funds (meaning funds specified in Article 131 of the Act on General Incorporated Association and General Incorporated Foundation (Act No. 48 of 2006); the same applies in Article 65, paragraph (1), item (ii)) and amount of net assets;
資本金又は基金（一般社団法人及び一般財団法人に関する法律（平成十八年法律第四十八号）第百三十一条に規定する基金をいう。第六十五条第一項第二号において同じ。）の額及び純資産額
name and location of business offices or offices;
営業所又は事務所の名称及び所在地
name of company director and company auditor (company director in cases of a company with audit and supervisory committee; company director and executive officer in cases of a company with nominating committee, etc.) or director and auditor;
取締役及び監査役（監査等委員会設置会社にあっては取締役、指名委員会等設置会社にあっては取締役及び執行役）又は理事及び監事の氏名
in cases of a company with accounting advisors, names of accounting advisors;
会計参与設置会社にあっては、会計参与の氏名又は名称
categories of funds transfer transaction analysis services (meaning the categories of business related to the acts set forth in the items of Article 2, paragraph (21); the same applies in Article 63-33, paragraphs (1) and (2) and Article 107, item (xix));
為替取引分析業の種別（第二条第二十一項各号に掲げる行為に係る業務の種別をいう。第六十三条の三十三第一項及び第二項並びに第百七条第十九号において同じ。）
name or trade name or other name and address of the financial institution, etc. that entrusts the funds transfer transaction analysis business to the person that intends to obtain a license, in connection with the funds transfer transactions carried out thereby; and
other particulars specified by order of the competent ministry.
その他主務省令で定める事項
The following documents must be attached to the written application for a license referred to in the preceding paragraph:
前項の許可申請書には、次に掲げる書類を添付しなければならない。
a document to pledge that the applicant does not fall under any of the items of paragraph (2) of the following Article;
次条第二項各号に掲げる要件に該当しない旨を誓約する書面
articles of incorporation;
定款
certificate of registered particulars;
登記事項証明書
business rules;
業務方法書
balance sheet and profit and loss statement;
貸借対照表及び損益計算書
documents stating the expected income and expenditure; and
収支の見込みを記載した書類
other documents specified by order of the competent ministry.
