Supplementary Provisions, Article 1Effective Date
第一条（施行期日）
This Act comes into effect on the date specified by Cabinet Order within a period not exceeding one year and three months from the date of promulgation; provided, however, that the provisions set forth in the following items come into effect on the dates specified respectively in those items:
この法律は、公布の日から起算して一年三月を超えない範囲内において政令で定める日から施行する。ただし、次の各号に掲げる規定は、当該各号に定める日から施行する。
The provision amending Article 3, paragraph (2) of the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures in Article 1, and the provision amending Article 9, paragraph (2) of that Act, and the provisions of Article 13, and Article 17, Article 19, and Article 20 of the Supplementary Provisions: the date of promulgation
the provisions in Article 2 amending Article 2, paragraph (7) of the Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures (limited to the part adding "except where, as of the date of the application under Article 16-2, paragraph (1), the person's age is less than the age specified by order of the competent ministry" after "is recorded," in that paragraph and the part replacing "Article 17, paragraph (5)" with "Article 17, paragraph (6)" in item (ii) of that paragraph), the provisions amending Article 16-2 of that Act, the provisions amending Article 17 of that Act, the provisions amending Article 18-2 of that Act, the provisions amending Article 38-8, paragraph (1) of that Act and the provisions amending Article 44 of that Act; the provisions of Articles 5 and 6 and Articles 8 through 12; the following Article; and the provisions of Articles 15, 16 and 18, Articles 22 through 25 and Article 27 of the Supplementary Provisions: the date specified by Cabinet Order within a period not exceeding one year and six months from the date of promulgation.
第二条中行政手続における特定の個人を識別するための番号の利用等に関する法律第二条第七項の改正規定（同項中「記載され、」の下に「第十六条の二第一項の申請の日において本人の年齢が主務省令で定める年齢に満たない場合を除き」を加える部分及び同項第二号中「第十七条第五項」を「第十七条第六項」に改める部分に限る。）、同法第十六条の二の改正規定、同法第十七条の改正規定、同法第十八条の二の改正規定、同法第三十八条の八第一項の改正規定及び同法第四十四条の改正規定並びに第五条、第六条及び第八条から第十二条までの規定並びに次条並びに附則第十五条、第十六条、第十八条、第二十二条から第二十五条まで及び第二十七条の規定 公布の日から起算して一年六月を超えない範囲内において政令で定める日
Supplementary Provisions, Article 15Transitional Measures Accompanying Partial Amendment of the Health Insurance Act, etc.
第十五条（健康保険法等の一部改正に伴う経過措置）
An Insurer (meaning the Insurer prescribed in Article 4 of the Health Insurance Act) may, in the case prescribed in the first sentence of Article 51-3, paragraph (1) of the Health Insurance Act as amended by the provisions of Article 5, if it finds it necessary, for the time being, notwithstanding the provisions of that paragraph, ex officio, deliver to an insured, as provided by the Order of the Ministry of Health, Labour and Welfare referred to in the second sentence of that paragraph, a document stating the matters specified by the Order of the Ministry of Health, Labour and Welfare referred to in that paragraph, or provide those matters by the electronic or magnetic means prescribed in that paragraph.
The provisions of the preceding paragraph apply mutatis mutandis to the delivery of documents and the provision by electronic or magnetic means under Article 28-2, paragraph (1) of the Mariners Insurance Act as amended by the provisions of Article 6, Article 22, paragraph (6) of the Act on Remuneration, etc. of Ministry of Defense Personnel as amended by the provisions of Article 8, Article 53-2, paragraph (1) of the National Public Officers Mutual Aid Association Act as amended by the provisions of Article 9, Article 9, paragraph (2) of the National Health Insurance Act as amended by the provisions of Article 10 (including as applied mutatis mutandis pursuant to Article 22 of that Act), Article 55-2, paragraph (1) of the Local Public Officers, etc. Mutual Aid Association Act as amended by the provisions of Article 11, or Article 54, paragraph (3) of the Act on Assurance of Medical Care for Elderly People as amended by the provisions of Article 12. In this case, any technical replacement of terms necessary is specified by Cabinet Order.
前項の規定は、第六条の規定による改正後の船員保険法第二十八条の二第一項、第八条の規定による改正後の防衛省の職員の給与等に関する法律第二十二条第六項、第九条の規定による改正後の国家公務員共済組合法第五十三条の二第一項、第十条の規定による改正後の国民健康保険法第九条第二項（同法第二十二条において準用する場合を含む。）、第十一条の規定による改正後の地方公務員等共済組合法第五十五条の二第一項又は第十二条の規定による改正後の高齢者の医療の確保に関する法律第五十四条第三項の規定による書面の交付及び電磁的方法による提供について準用する。この場合において、必要な技術的読替えは、政令で定める。
Supplementary Provisions, Article 18Transitional Measures Accompanying the Partial Amendment of the Act on Assurance of Medical Care for Elderly People
第十八条（高齢者の医療の確保に関する法律の一部改正に伴う経過措置）
With regard to the health insurance card or health insurance certificate of a person who, at the time the provisions of Article 12 come into effect, is actually issued a health insurance card or health insurance certificate by the Association of Medical Care Systems for the Elderly Aged 75 and older, in the case where that person receives medical treatment from a medical institution providing services covered by health insurance, etc. on or after the item (ii) Effective Date or receives designated home-nursing from a designated home-nursing provider, the provisions then in force continue to govern during the period in which that health insurance card or health insurance certificate is deemed to be valid under the provisions of the Act on Assurance of Medical Care for Elderly People as it stood before the amendment by the provisions of that Article (including orders based thereon) (or, where the last day of that period falls on or after the day following the day on which one year has elapsed counting from the item (ii) Effective Date, for one year counting from the item (ii) Effective Date).
第十二条の規定の施行の際現に後期高齢者医療広域連合から被保険者証又は被保険者資格証明書の交付を受けている者が、第二号施行日以後に保険医療機関等から療養を受ける場合又は指定訪問看護事業者から指定訪問看護を受ける場合における当該被保険者証又は被保険者資格証明書については、同条の規定による改正前の高齢者の医療の確保に関する法律（これに基づく命令を含む。）の規定により当該被保険者証又は被保険者資格証明書が効力を有するとされた間（当該期間の末日が第二号施行日から起算して一年を経過する日の翌日以後であるときは、第二号施行日から起算して一年間とする。）は、なお従前の例による。
Supplementary Provisions, Article 19
第十九条
The Association of Medical Care Systems for the Elderly Aged 75 and older may carry out, even before the item (ii) Effective Date, acts such as the enactment or amendment of an Ordinance of the Association that are necessary for the enforcement of the Act on Assurance of Medical Care for Elderly People (including orders based thereon) as amended by the provisions of Article 12.
後期高齢者医療広域連合は、第十二条の規定による改正後の高齢者の医療の確保に関する法律（これに基づく命令を含む。）の施行のために必要な条例の制定又は改正その他の行為については、第二号施行日前においても行うことができる。
Supplementary Provisions, Article 20Delegation to Cabinet Order
第二十条（政令への委任）
Beyond what is provided for in these Supplementary Provisions, the transitional measures necessary in connection with the enforcement of this Act (including transitional measures concerning penal provisions) are specified by Cabinet Order.
この附則に定めるもののほか、この法律の施行に関し必要な経過措置（罰則に関する経過措置を含む。）は、政令で定める。
