A person who seeks to use the surname they were using at the time of divorce pursuant to the provisions of Article 767, paragraph (2) of the Civil Code (including as applied mutatis mutandis pursuant to Article 771 of the relevant Code) must state this and the date of their divorce in the written notification and submit it.
