A person who intends to obtain a license referred to in paragraph (1) of the preceding Article must, pursuant to the provisions of Cabinet Office Order, submit a written application for license containing the following particulars to the Prime Minister:
前条第一項の免許を受けようとする者は、内閣府令で定めるところにより、次に掲げる事項を記載した免許申請書を内閣総理大臣に提出しなければならない。
trade name or other name and address;
商号又は名称及び住所
name and location of business offices or offices;
営業所又は事務所の名称及び所在地
name of company director and company auditor (company director in cases of a company with audit and supervisory committee; company director and executive officer in cases of a company with nominating committee, etc.) or director and auditor;
取締役及び監査役（監査等委員会設置会社にあっては取締役、指名委員会等設置会社にあっては取締役及び執行役）又は理事及び監事の氏名
in cases of a company with accounting advisors, names of accounting advisors; and
会計参与設置会社にあっては、会計参与の氏名又は名称
other particulars specified by Cabinet Office Order.
その他内閣府令で定める事項
The following documents must be attached to the written application for a license referred to in the preceding paragraph:
前項の免許申請書には、次に掲げる書類を添付しなければならない。
a document to pledge that the applicant does not fall under any of the items of paragraph (2) of the following Article;
次条第二項各号に掲げる要件に該当しない旨を誓約する書面
articles of incorporation;
定款
certificate of registered information;
登記事項証明書
business rules;
業務方法書
balance sheet and profit and loss statement;
貸借対照表及び損益計算書
documents stating the expected income and expenditure; and
収支の見込みを記載した書類
other documents specified by Cabinet Office Order.
