A prefecture, if it finds it necessary for the preparation of the medical care plan or for the execution of activities based on the medical care plan, may request the delivery of any necessary information from the municipalities and other public offices, medical insurers provided for in Article 7, paragraph (7) of the Long-Term Care Insurance Act (referred to as "medical insurers" in Article 30-14, paragraph (1) and Article 30-18-5, paragraph (1)), or establishers, administrators or founders of medical institutions, including information concerning the functions of medical institutions within the boundaries of the relevant prefecture.
都道府県は、医療計画を作成し、又は医療計画に基づく事業を実施するために必要があると認めるときは、市町村その他の官公署、介護保険法第七条第七項に規定する医療保険者（第三十条の十四第一項及び第三十条の十八の五第一項において「医療保険者」という。）又は医療提供施設の開設者、管理者若しくは設置者に対し、当該都道府県の区域内における医療提供施設の機能に関する情報その他の必要な情報の提供を求めることができる。
